Our Western Colorado team serves clients across Mesa, Delta, Montrose, Garfield, and Ouray counties. We provide a mix of private pay, Veterans Administration, Medicaid, and long-term care services and are one of the largest and fastest-growing home care providers in the region.

We are seeking a Director of Nursing who wants to do more than maintain compliance. We are looking for a clinical leader who can help build systems, support our team, improve quality, and contribute to the continued growth of a mission-driven organization.

Position Summary

The Director of Nursing serves as the agency's clinical leader and is responsible for ensuring safe, compliant, and high-quality care delivery across all service lines.

This role combines clinical oversight, regulatory compliance, quality assurance, nurse leadership, and operational collaboration. The DON works closely with leadership, Care Experience Managers, schedulers, recruiters, and caregivers/home health aides to ensure clients receive exceptional care while supporting the continued growth and scalability of the organization.

This is an excellent opportunity for a nurse who enjoys leadership, problem-solving, process improvement, and mentoring others while maintaining direct involvement in client care and clinical decision-making.

Key Responsibilities

Clinical Leadership

Provide leadership, guidance, and supervision to agency nursing staff
Support clinical decision-making for complex client situations
Ensure care plans, assessments, and supervisory visits meet agency standards
Promote a culture of clinical excellence, accountability, and client-centered care

Compliance & Quality

Ensure compliance with Colorado regulations, nursing practice standards, payer requirements, and agency policies
Maintain survey readiness and support regulatory inspections
Audit clinical documentation and identify opportunities for improvement
Participate in Quality Assurance and Performance Improvement (QAPI) activities
Monitor clinical trends, incidents, and quality indicators

Clinical Operations

Oversee completion of nursing assessments, supervisory visits, and clinical documentation
Coordinate nursing resources to meet client needs throughout the service area
Assist with development and refinement of clinical workflows and systems
Collaborate with operations leadership to improve efficiency and service delivery

Veterans Administration & Payer Programs

Serve as the clinical liaison for Veterans Administration programs and other payer relationships
Ensure required clinical documentation is completed accurately and on time
Support growth initiatives involving VA and other clinically driven service lines

Training & Development

Provide coaching and support to nurses, caregivers, and care management staff
Assist with clinical education and competency development
Identify training needs and opportunities for improvement

Cross-Functional Leadership

Partner closely with Care Experience Managers regarding assessments, care plans, client concerns, and changes in condition
Collaborate with scheduling and operations teams to address service challenges
Work alongside executive leadership on strategic initiatives, growth, and operational improvements

Qualifications

Active Colorado Registered Nurse (RN) license in good standing
Minimum three years of nursing experience
Previous supervisory or leadership experience preferred
Home care, home health, hospice, case management, senior care, or community-based care experience strongly preferred
Strong understanding of clinical documentation, compliance, and regulatory requirements
Excellent communication, leadership, and problem-solving skills
Ability to balance clinical oversight with operational collaboration
Comfortable using electronic documentation, modern software/IT platforms, and electronic healthcare records management systems (e.g. WellSky).
