Full Job Description

Are you driven by a passion to help others? Are you someone who is objective, confident team player and problem solver?

If yes, Lakemary is looking for a Case Manager for our Adult's Service Program. This position shall assist the person and the person's support network to identify, select, obtain, coordinate, and use both paid services and natural supports that are available to enhance the person's independence, integration, and productivity consistent with the person's capabilities and preferences as outlined in the person's person-centered support plan. This role functions under dual general supervision of the Coordinator of Social Services.

Case Manager Responsibilities:

Identify the person's needs, their preferred lifestyle, and the resources that are available, through formal and informal evaluation methods.
Arranging for professional evaluations or assessments.
Arranging for referral or placement packets to be compiled or completed.
Build a rapport with individuals served and families to understand their needs, preferences, supports, and services
With the participation of the person served and their support network, annually develop and facilitate the person-centered support plan and as applicable any related service or support plans
Build upon assessment information to assist the person served in meeting their needs and achieving their preferred lifestyle.
Provide assistance to the person served in becoming knowledgeable about the community services and support options available and assist them in obtaining access.
Provide assistance to the person served in receiving information regarding their rights.
Ensure the active participation of the person served, and work with them (or their legal representative) and others to develop goals and identify a course of action to respond to the assessed needs.
Knowledgeable of the HCBS MR/DD Manual and DD Reform Articles 63 and 64.

An ideal candidate for this position has:

Utilization of time management skills.
Microsoft Word and Excel knowledgeable.
Interpret, apply and explain applicable rules and regulations.
Maintain confidentiality of individual served.
Respond to requests and inquiries in a timely professional and timely manner.
Understand and follow instructions.
Establish and maintain effective working relationships.

Case Manager Qualifications:

Bachelor's degree or additional full-time experience in the field of developmental disabilities services, which may be substituted for the degree at the rate of six months of full-time experience for each missing semester of college.
For a bachelor's degree, must have six months of full-time experience in the field of human services.
For a non-Degreed, must meet all the requirements under Education plus a minimum of two years related experience. One plus years of cases management experience preferred.
Knowledge of office equipment & personal computers.
Utilization of time management skills
Maintain confidentiality of individual served.
Ability to multi-task and meet deadlines
Record, file, and transmit information.

Other Job Details:

Day hours 7am-3pm Monday through Friday
Wage: $21.00
