Physician / Medical Director (Hybrid, Contracted)

Schedule: Hybrid; 4-5 hours per week (rotating on call schedule)
On-Site Requirement: One consistent on-site shift per month, Monday–Friday between 7:30 AM–11:30 AM (during OTP hours)
Position Summary
The Physician/Medical Director is responsible for conducting substance use disorder (SUD) evaluations, prescribing and monitoring medications, and providing high-quality medical services within CASA-Trinity’s programs. In collaboration with the clinical team, this role ensures person-centered, evidence-based care and contributes to organizational leadership through clinical oversight, problem solving, strategic planning, and performance improvement.
This is a contracted, hybrid position requiring one consistent on-site shift per week.
Essential Job Functions Physician Responsibilities
- Screen, evaluate, and treat common substance use disorders; refer to mental health providers as appropriate.
- Provide clinical oversight to mid-level practitioners, interns, and medical staff.
- Substantiate diagnoses, assess detox needs, determine MAT appropriateness, and treat medical ailments and chronic conditions.
- Perform common office-based procedures.
- Review and sign treatment plans, lab slips, health forms, and participate in incident reviews and quality assurance activities.
- Perform and/or review medical assessments and physical examinations when needed.
- Oversee development and revision of medical policies, procedures, specialized medications, and emergency care protocols.
- Ensure high-quality health care services related to stabilization and rehabilitation.
- Respond flexibly to changing clinical needs based on patient acuity and program census.
- Prescribe and administer buprenorphine, Suboxone, and naltrexone for alcohol and opioid use disorders.
- Communicate with external care providers/referral sources to support coordinated care.
- Participate in professional development and clinical supervision.
- Complete timely and compliant medical documentation in accordance with NY regulations.
- Meet productivity expectations and reduce no-show rates through engagement strategies.
- Collaborate as an active member of an interdisciplinary team.
- Participate in scheduled on-call coverage.
- Maintain QHP status and perform other duties as assigned.
Medical Director Responsibilities
- Ensure all clinical care meets best practices and established clinical standards.
- Provide leadership in developing treatment protocols and standards of care.
- Offer consultation to staff regarding medical issues and medications.
- Prescribe or authorize medications administered within the facility or by other qualified prescribers.
- Oversee development of policies and procedures related to stabilization and rehabilitation services.
- Ensure program standards and philosophy remain consistent and clinically appropriate.
- Consult with specialists and refer patients as appropriate.
- Ensure admission assessments, labs, and ongoing care are timely, accurate, and appropriate.
- Provide and/or supervise all medical care delivered to patients.
- Participate in the hiring and evaluation of medical staff; ensure clinical competency and professional development.
Knowledge, Skills, and Abilities
- Excellent written, verbal, and interpersonal communication skills.
- Proficiency with Electronic Medical Records (EMR).
- Strong analytical, assessment, and problem-solving skills.
- Ability to prepare reports, write professional correspondence, and contribute to policy/procedure development.
- Ability to prescribe Suboxone.
- Strong clinical diagnostic and treatment skills.
- Demonstrated independence, initiative, leadership, and professionalism.
- Ability to work collaboratively with staff, administration, clients, board members, and community partners.
Physical Demands & Work Environment
- Ability to sit, stand, walk, climb stairs, use standard office/medical equipment, and occasionally lift up to 20 lbs.
- Requires visual acuity, sound perception, and fine motor skills for clinical tasks.
- Work is performed in a tobacco- and alcohol-free environment.
- Must be able to manage stressful interactions, including clients in crisis or with behavioral challenges.
- May be exposed to illness; proper self-protection is required.
Qualifications
- Board Certified/Board Eligible in Internal Medicine, Family Practice, or Addiction Medicine strongly preferred.
- Minimum of 2 years of experience in addiction medicine OR a strong demonstrated interest in the field.
- Active DEA license.
- New York State medical license required; Pennsylvania license preferred.
- DATA 2000 waiver (buprenorphine certification) required.

Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
