Full Job Description

Care Navigation Coordinator

POSITION OVERVIEW

The Care Navigation Coordinator works directly with patients every day — by phone and telehealth — across 6–8 rural Nebraska partner facilities as part of Encounter Telehealth’s RHTP 4.4A grant program, serving approximately 150–250 participants through July 31, 2027, with final facility and participant targets set by the approved DHHS workplan.

Daily work includes:

Calling and video-connecting with patients and families to coordinate appointments, follow up after discharges, and support medication reconciliation
Enrolling participants into the chronic disease behavioral health management program (enrollment targets per the approved DHHS workplan) and administering PHQ-9, GAD-7, and SDOH screenings
Scheduling telehealth session appointments, and working with facility contacts — administrators, discharge planners, and nursing staff — to keep sessions running smoothly
Tracking referrals to completion in a closed-loop system and connecting patients to community resources such as transportation, food access, and primary care
Documenting all patient interactions in Tebra to support quarterly Nebraska DHHS grant reporting

The Coordinator also delivers structured 6-week chronic disease self-management workshops via telehealth and expands behavioral health access to facility staff, families, and the broader rural community. All work is delivered remotely — no facility travel required; the facility’s only ongoing obligation is patient referral and minimal session facilitation. This is a builder role: shaping workflows, earning trust by phone, and making sure no patient falls through the cracks between discharge, referral, and behavioral health care.

CORE RESPONSIBILITIES

A. Care Navigation & Patient Engagement

Primary navigation contact. Serve as the main point of contact for enrolled participants across all partner facilities — 6–8 rural Nebraska sites — (skilled nursing facilities, assisted living communities, critical access hospitals, rural health clinics, and FQHCs), managing appointment coordination, medication reconciliation support, insurance literacy, disease management education and community resource linkage through a closed-loop referral tracking system.
Enrollment. Onboard participants into the chronic disease behavioral health management program; enrollment targets per the approved DHHS workplan.
Transition-of-care outreach. Conduct follow-up to reduce gaps between discharge, referral, and behavioral health engagement.
Site relationships. Build and maintain trust-based relationships with DONs, Social Services Directors, and nursing leads at all assigned rural partner sites — the consistent human face of Encounter’s program at each location.
Telehealth coordination. Coordinate with facility-designated Telehealth Coordinators (SSDs or nursing designees) to support smooth session facilitation and equipment use.
Crisis escalation. Implement crisis escalation protocols when PHQ-9 or GAD-7 screening flags safety concerns, following LPN scope-of-practice guidelines and connecting patients to appropriate clinical resources.
Assessment Administration. Administer several different SDOH related assessments to participants in the care navigation program

B. Community Education & Outreach (CHW Function)

Workshop delivery. Plan and deliver structured chronic disease self-management workshops in 6-week cycles addressing behavioral health barriers to medication adherence, care plan follow-through, and self-management — targeting 150+ participants across 3+ cycles by July 2027.
Community access. Expand behavioral health access beyond facility residents to facility staff families and broader rural community members, building and operationalizing community referral pathways by October 31, 2026.
Community partnerships. Develop and maintain relationships with rural primary care providers, schools, Area Agencies on Aging, transportation organizations, and food access resources — executing MOUs with 3+ community organizations per service area.
Individual & group education. Deliver sessions emphasizing the connection between mental health and chronic disease outcomes (diabetes, CVD, COPD).
Screening support. Assist with PHQ-9 and GAD-7 screening protocol implementation and provide participant education on behavioral health screening and referral pathways.

C. Telehealth Equipment & Site Coordination

Equipment deployment. Coordinate rollout of telehealth equipment kits at all 6 -8 partner sites by July 31, 2026, including HIPAA-compliant configuration and facility coordinator training.
Ongoing support. Serve as the operational point of contact for equipment troubleshooting throughout the grant period.
Scheduling. Manage participant scheduling and session logistics across all 6 sites, ensuring PMHNP-to-facility provider continuity.

Pay: $55,000.00 - $60,000.00 per year
