About the Role

Are you a compassionate healthcare professional who excels at problem-solving and patient care? We are looking for a dedicated Patient Navigator to serve as a crucial bridge between our patients, Primary Care Physicians (PCPs), specialists, and administrative teams.

In this role, you will ensure seamless care coordination by obtaining medical records, managing specialist referrals, securing test authorizations, and empowering patients every step of the way. If you thrive in a fast-paced medical environment where clear communication and empathy make a daily impact, we want to hear from you!

Key Responsibilities

Care Coordination & Access: Assist patients with scheduling specialist visits, generating referrals, obtaining prior authorizations, and guiding them through insurance and patient portal enrollment.
Patient & Team Communication: Answer incoming phone calls, monitor voicemails, EHR messages, and emails in a timely manner. Relay critical information clearly via phone, text, mail, or EHR documentation.
Records Management: Obtain outside medical records prior to upcoming appointments to ensure doctors have complete information. Triage, abstract, scan, and route incoming reports and forms.
Patient Education & Support: Partner with the medical team to assist with patient education, answering questions with warmth and strong customer service.
EHR Documentation: Maintain an efficient flow of documents into and out of the Electronic Health Records system while keeping all data accurate and up to date.

What We’re Looking For

EHR Proficiency: Direct experience accessing, navigating, and performing accurate data entry in Electronic Health Record systems.
Customer Service Excellence: Exceptional communication skills paired with deep empathy, active listening, and a patient-first mindset.
Strong Problem-Solving: Ability to troubleshoot insurance authorization hurdles, missing medical records, and scheduling conflicts with ease.
Organization & Efficiency: Excellent time management and the ability to prioritize incoming requests, records, and messages in a high-volume setting.
