Full Job Description

Founded in 1977, Mariposa Women and Family Center (“Mariposa”) is a non-profit organization committed to empowering women, men, children, and families through comprehensive support services, including mental health and substance use counseling, as well as enhanced care management. We provide a safe and nurturing environment where individuals can access resources, education, and advocacy to foster personal growth, strengthen family bonds, and achieve their full potential. We are currently seeking a passionate and dedicated Housing Navigator to assists individuals experiencing homelessness and those re-entering society in securing safe and stable housing. The Housing Navigator provides comprehensive support services to guide clients through the housing process, with the goal of achieving long-term stability and successful reintegration into the community.

Essential Functions:

Conduct client intake and utilize the Clarity Homeless Management Information System (HMIS) for inputting and maintaining accurate and up-to-date client information.
Leverage HMIS functionalities to streamline intake procedures, track client progress, generate reports for program evaluation and reporting purposes, and enter all ACES participants into the Coordinated Entry system for housing opportunities.
Complete in-depth assessments to gather information on client demographics, housing history, barriers to housing stability, income, employment status, physical and mental capacity and needs, and other relevant factors.
Collaborate with clients to develop personalized housing plans based on assessment findings and client input, outlining actionable steps and timelines for achieving housing stability.
Assist clients in identifying suitable housing options based on their preferences, budget, and eligibility criteria.
Provide guidance and support throughout the application and leasing process, including assistance with paperwork, negotiations, and landlord communication.
Coordinate with landlords and property managers to secure housing placement and negotiate lease agreements on behalf of clients.
Serve as a designated case manager, providing ongoing support and advocacy for clients.
Conduct regular check-ins to monitor progress, address challenges, and adjust housing plans as needed.
Connect clients with supportive services, including employment assistance, financial counseling, substance abuse treatment, and mental health counseling.
Coordinate with partner agencies and community organizations to access additional resources and referrals, as necessary.
Maintain accurate records of clients' demographics, housing outcomes, and service utilization.
Generate regularly scheduled reports to track program performance, outcomes, and areas for improvement.
Collect feedback from clients and stakeholders to inform program evaluation and continuous quality improvement efforts.
Maintain confidentiality and privacy of participants in accordance with HIPAA regulations and program policies.

Qualifications/Requirements:

High school diploma or equivalent education or experience.
Working knowledge of local housing resources, tenant rights, and barriers faced by homeless or at-risk individuals.
Proficiency in case management software and data entry.
Problem-solving skills and crisis intervention experience
Willingness to work primarily in the field, meeting clients or accompanying them to appointments as needed.
Flexibility to work some evenings or weekends based on client needs and program demands.
Excellent communication, organizational, and interpersonal skills.
Ability to effectively manage a caseload, prioritize tasks, and provide compassionate support.
High level of professionalism with demonstrated ability to provide excellent customer service via phone, email, and in-person interactions.
Ability to handle sensitive information with integrity and maintain high level of confidentiality.
Ability to work independently and collaboratively, maintaining professionalism in line with agency and county policies.
Respectful and approachable, with high standards for integrity and confidentiality.
Detailed-oriented and organized, with the ability to be flexible.
Proficient with Microsoft Office including but not limited to Outlook, SharePoint, Teams, Calendar, etc.

This position is subject to background clearance, including Live Scan

Pay: $30.00 per hour
