Hello

We're Aion Therapy, an outpatient behavioral health practice in Portland. About 1,000 people count on us for care, and roughly 90% of them are covered through Oregon Health Plan CCOs. We bill around $5 million a year.

We're looking for someone to take the whole billing operation and make it theirs.

What this job really is

You'd own the full accounts receivable cycle — charge entry through final payment, denials, appeals, secondary billing, and month-end reporting. You'd be the person who knows where every dollar is and why.

We want to be upfront about two things:

This is a hands-on role. You won't have a team under you. If you love being the person who actually does the work and sees it through, you'll like it here. If you're hoping to move into managing people, this probably isn't your next step.

Our payer mix is demanding. Working with CCOs means eligibility that shifts without warning, retroactive COB recoveries that land in your lap, and coordination-of-benefits puzzles that take real skill to solve. If you've done this work, you know exactly what we mean — and you're probably the person we're looking for.

In exchange, you get genuine autonomy, a practice that will listen when you say something needs to change, and colleagues who understand that clean billing is what keeps the doors open for our clients.

Day to day

Claims and AR

Own the revenue cycle end to end: eligibility verification, charge review, claim submission, payment posting, denials, appeals, and patient balances
Submit and track claims to OHP CCOs, commercial payers, and Medicare
Work the aging report each week — you'll decide what gets attention and when
Keep an eye on timely filing deadlines across every payer so nothing slips away

OHP and coordination of benefits

Verify OHP eligibility and CCO assignment at intake and on an ongoing basis, catching plan changes before they become denials
Handle coordination of benefits: identify the right primary payer, sequence claims correctly, and bill secondary with accurate primary EOB and ERA data
Untangle COB denials, eligibility mismatches, and retroactive recoveries with CCOs and OHA
Stay on top of CCO-specific requirements — authorizations, encounter data, and each plan's particular quirks

Credentialing and payer relationships

Keep clinician credentialing and enrollment current across CCOs and commercial payers
Be our main point of contact with payer provider relations

Reporting

Produce monthly AR aging, collection rate, denial rate by reason, and days-in-AR reporting
Spot trends and payer-specific problems early, while they're still small
Support month-end close alongside our bookkeeper

Making things better

Document billing workflows so the knowledge doesn't live only in your head
Help clinical staff understand the documentation and coding details that affect claims — teaching, not policing
Tell us what's broken and what you'd do about it. We'll listen.

What we're hoping you bring

Four or more years in medical billing, including at least two running a full AR cycle on your own
Real Oregon Health Plan and CCO billing experience. Given our payer mix, this is the one thing we can't train around.
Strong coordination of benefits and secondary billing skills — you can talk through payer sequencing without looking it up
Denial management and appeals experience, including writing appeals that actually get paid
Comfort with volume; 1,000+ active clients doesn't faze you
Fluency with a practice management or EHR billing module and clearinghouse workflows
Self-direction. Working remotely as the only biller means setting your own priorities and following through without someone checking in.

Nice to have, not required:

Behavioral health billing specifically (90791, 90832–90838, 90846/90847, group and H-codes)
Familiarity with several Oregon CCOs — Health Share, CareOregon, Trillium, PacificSource Community Solutions
CPB, CPC, or CRCR certification
Credentialing and payer enrollment experience
Experience building or cleaning up a billing process, not just keeping one running
