JOB TITLE: Housing Case Manager

DEPARTMENT: Washington County Supportive Housing Services (SHS)

REPORTS TO: SHS Program Manager

FSLA: Non-Exempt (Hourly)

POSITION CLASS: Regular Full-Time

OVERVIEW

The SHS Housing Case Manager works under the supervision of the SHS Program Manager to provide high quality Housing Case Management Services to participants currently experiencing homelessness and housing instability and are eligible for supportive housing funded through the Metro Supportive Housing Services in Washington County. This position is primarily field based but could also be co-located or site based. At times visits may be required in the participants’ home, shelter or encampment.

ESSENTIAL DUTIES & RESPONSIBILITIES

Provide a participant centered approach and excellent customer service that is sensitive to the challenges of homelessness, including medical and behavioral health issues that face them, in their efforts to move into and maintain permanent supportive housing.

Assist participants in their transition from homelessness to permanent housing.
Assist households at the beginning of the housing stabilization process, including flexible services to meet the individual needs of participants.
Outreach and engagement including processing referrals from Community Connect.
Establish rapport with participants.
Provide a “screening in” philosophy.
Conducts face to face screening and obtain consents.
Determine program eligibility and process intake/enrollment.
Maintain compliance with Easterseals Oregon’s policies and procedures and the Supportive Housing Services program rules and regulations.
Provide linkage to other needed providers and services.
Conduct intake enrollment and enroll into HCMS.
Assist with temporary housing assistance until permanent housing can be secured.
Ensure all documentation is prepared and entered into the Homeless Management Information System (HMIS)
Assist at all levels of move-in to permanent housing; document ready, housing authority applications and other paperwork, coordination of move-in and orientation of new tenant to their unit/building.
May conduct home visits, shelter and/or camp visits as needed.
Conduct assessments, develop and implement individualized case management services plan in collaboration with the participant including needs, goals, steps, timeframes, and disposition of each goal as it is met or changed
Ensure access to health, mental health, and substance use disorder services.
Connect participants with community resources and supportive services for which they might be eligible, such as transportation, childcare, and legal services.
Ensure connection with medication and treatment regimens.
Provide housing location services and educate participants on tenant rights and responsibilities.
Educate participants on the appropriate use of crisis intervention services versus 911 emergency calls, etc.
Maintain accurate, up-to-date records and case files and report regularly to the HCMS Program Manager. Data entry and collection for reporting and record keeping must be on-going, complete, and accurate.
Maintain participant confidentiality and perform job responsibilities in accordance with the Easterseals Oregon’s personnel policies.
Maintain Professional Development growth.

OTHER RESPONSIBILITIES

Attend approximately three meetings per day within the Washington County area, totaling around 50 meetings per month.
Use a reliable personal vehicle with valid insurance to travel and provide services to participants throughout the county. You will receive mileage reimbursement in accordance with federal guidelines, currently at $0.70 per business mile.
Minimum personal auto insurance requirement is $100,000/$300,000.
Attend all required Easterseals Oregon and HCMS meetings and trainings.
Perform all other duties as assigned by the HCMS Program Manager or Program Director.

You will spend approximately:

50% of your time meeting with participants in Washington County, engaging with them in their communities and homes.
25% of your time working from the office, completing documentation, coordinating services, and collaborating with team members.
25% of your time performing additional tasks, which may occur either in the office or in the field. These tasks may include administrative duties, outreach activities, training sessions, or other responsibilities as assigned.

QUALIFICATIONS

COVID-19 Vaccination required; accommodation may be available for medical and religious purposes.
Associate degree in a human service or related field and Minimum 2 years of experience providing services related to similar population.
Bachelor’s degree in human services or related field and Minimum 1 year of experience providing related services to similar population
Lived experience can be used in place of educational requirements.
Knowledge of or ability to learn the Homeless Management Information Systems (HMIS)
Experience working with participants with mental illness, chronic health issues, and substance use disorders
Experience in the following areas: chronic homelessness, outreach and engagement strategies, housing navigation, best practice models, mental health and substance use disorder services, crisis intervention, suicide assessment and prevention, affordable housing and public benefits applications, housing and landlord/tenant rights, eviction prevention, etc.
Working knowledge of Microsoft Office products, including Word and Excel.
A valid Oregon driver’s license and insurance.
Organized and detailed oriented; ability to manage multiple cases efficiently and effectively.
Passion for ending homelessness.
Ability to work as a member of a team and independently.
Strong people skills.
Ability to work effectively with people of varying racial, ethnic, cultural, educational, and socio-economic backgrounds.
Successful completion of background check and reference check required.
