Full Job Description

The Senior Housing Program is a permanent supportive housing program located in San Francisco that serves primarily low-income seniors who have experienced homelessness and are living with complex and long-term social, mental health, substance use, and medical challenges.

Qualifications:

· BA or BS degree and 2 years’ experience in social work, preferably in the non-profit or public sector; or equivalent combination of education and lived experience.

Bachelor’s degree in social work or related field preferred.

· Minimum 2 years’ case management experience working with diverse populations.

· Experience working with and sensitivity to issues affecting older adults with complex and long-term social and medical issues including people recovering from the trauma of domestic violence, substance use, marginal housing/employment, and mental health concerns.

· Experience working collaboratively with other agencies, and the ability to relate professionally to other service providers.

· Experience working with disabled older adults / seniors preferred.

· Experience providing social services in a residential setting.

· Have good writing, computer, and analytical skills.

Competent computer skills: MS Windows, MS Office, MS Outlook

Essential Duties and Responsibilities:·

Provide comprehensive case management and social service coordination, including crisis intervention, client advocacy, counseling, referrals, and connecting residents with community resources to support independent living.
Develop and maintain client service plans and documentation while collaborating with residents, families, property management, and community agencies to coordinate care.
Foster a positive residential community by mediating conflicts, supporting tenant success in independent living, and serving as a resource to property management staff.
Organize resident engagement activities, assist with Food Bank distribution, participate in meetings and training, and support additional program initiatives as needed.

Pay: $29.50 - $30.00 per hour
