The Senior HVRP Case Manager serves as the lead administrative and compliance-focused case manager for SALT Outreach's Homeless Veterans' Reintegration Program (HVRP). This position specializes in participant eligibility determination, documentation review, compliance, and quality assurance while providing advanced case management support for veterans with complex service needs. Working closely with the Veteran Services Supervisor, the Senior HVRP Case Manager strengthens program operations by ensuring participant files meet Department of Labor requirements, supporting accurate reporting, coordinating complex referrals, and assisting with VGRS documentation and compliance activities.
Responsibilities and Duties:
The functions of this position include:
- Participant Eligibility & Enrollment
- Review participant eligibility documentation.
- Verify required HVRP eligibility criteria.
- Complete enrollment reviews prior to approval.
- Ensure participant files contain required documentation.
- Assist staff with resolving documentation deficiencies.
- Documentation & Compliance
- Maintain participant files in accordance with DOL regulations.
- Complete and review VGRS documentation.
- Monitor documentation quality across the HVRP team.
- Ensure timely case notes and service documentation.
- Conduct internal file reviews.
- Prepare documentation for monitoring visits and audits.
- Assist with grant reporting requirements.
- Maintain confidential participant records.
- Advanced Case Management
- Carry a smaller caseload consisting of participants with higher service needs.
- Coordinate referrals.
- Assist with benefit enrollment.
- Address barriers.
- Coordinate with VA partners.
- Assist outreach staff with complex situations.
- Program Support
- Serve as the documentation resource for HVRP staff.
- Train staff on documentation requirements.
- Assist supervisor with quality assurance.
- Monitor participant progress.
- Identify missing documentation.
- Help improve workflows.
- Reporting
- Assist with monthly and quarterly performance reports.
- Track participant outcomes.
- Monitor enrollments.
- Assist with DOL reporting.
- Coordinate with Grants & Data.

Qualifications:
Requirements:
- Bachelor's degree in social work, human services, healthcare or related field.
- Excellent verbal and written communication skills
- Strong problem-solving and interpersonal skills
- Ability to work independently in the field and collaborate within multidisciplinary teams.
- Organized, accurate, thorough, detail-oriented, and able to monitor work for quality
- Must be able to stand and walk for prolonged periods
- May need to walk/stand for up to 8 hours at a time with a standard break
- Intermediate level with MS Excel, MS Word, and Google Docs.
- Must have a valid driver’s license and personal vehicle.
Preferences
- Lived experience as a veteran or prior military service.
- Experience with compliance/documentation.
- Experience in working with people experiencing homelessness.
- Knowledge of VA services and veteran benefits systems.
- Cultural competency and sensitivity toward veteran experiences, including PTSD and substance use.
Physical Requirements
- Ability to lift up to 50 pounds

- Must hear and see well (either naturally or with correction)
- Must be able to bend, stoop, squat, kneel, push, pull, and reach overhead
- Work in a fast-paced environment with frequent interactions with customers

- Work in various weather conditions, including heat, cold, and rain
- Stand and walk continuously for up to 8-10 hours during a shift while providing customer care
- Considerable amount of time using a telephone and/or computer
- Answer phone calls and communicate with colleagues and clients verbally throughout the day
- Use hands and fingers for typing and operating office equipment, such as printers and scanners
- Use hands and fingers for precise mouse movements and keyboard shortcuts
- Review and analyze printed materials for accuracy and quality
- Attend meetings and collaborate with team members in various settings
