Job Description

General Functions:

Provides counseling and case management services to clients of the division and their families. Responsible direct service work in residential and non-residential programs.

Work is performed under the supervision of a Senior Children's Services Counselor, Counseling Services Supervisor or a Youth & Family Services Program Manager and is reviewed through file reviews and service verifications, conferences, records, reports, observation, and results achieved.

Representative Duties/Assignments:

Duties may vary based on area of assignment. The following are representative duties, and it is not the intent to capture all duties performed by this classification.

Provides case management and counseling services to children and/or their families in home, group residential and/or office settings.
Serves as liaison and coordinates services within and between the division, courts, other agencies and practitioners for assigned cases.
Maintains case records in accordance with program and division policies and procedures.
Conducts client screening and intake processes
Develops and implements an appropriate case plan for clients including measurable goals in accordance with division policies and procedures.
May serve on an on-call rotation.
Initiates and/or participates in team staffing and case conferences for assigned cases.
May transport clients to appointments with other agencies and practitioners.
Prepares and submits reports and data as required. Performs other responsibilities required to provide the specific services of the assigned program.
May provide direction and supervision to paraprofessional and direct care staff.
Performs other related duties as required.

Minimum Qualifications:

Bachelor's Degree in the social or behavioral sciences or a closely related field and one year of experience in social or human services.

A Master's Degree in counseling, social or behavioral sciences or related field

Youth & Family Services:

May be required to possess and maintain a valid Florida Driver's license and provide own vehicle for job related duties.

Must be able to demonstrate intermediate to advanced proficiency in word processing, preferably Microsoft Word, and email functions.
