Full Job Description

SUMMARY: The Resident Care Coordinator is responsible for overseeing the delivery of resident care services within the Memory Care community. This position coordinates care plans, supervises caregiving staff, ensures compliance with regulatory requirements, and promotes the highest quality of life for residents living with dementia and other cognitive impairments. The Resident Care Coordinator works closely with residents, families, healthcare providers, and community leadership to ensure individualized, person-centered care.

ESSENTIAL DUTIES AND RESPONSBILITIES:

Coordinate and monitor daily resident care services in the Memory Care neighborhood.
Conduct resident assessments and assist with development, implementation, and updates of individualized service plans.
Monitor residents for changes in physical, mental, emotional, and behavioral status and communicate concerns to appropriate healthcare professionals.
Ensure resident care needs are met in accordance with physician orders, community policies, and state regulations.
Participate in resident move-ins, assessments, and care conferences.

Supervise and support caregivers and medication technicians during assigned shifts.
Assist with scheduling and staffing to ensure adequate coverage and continuity of care.
Provide coaching, training, and performance feedback to direct care staff.
Monitor staff compliance with infection control, safety procedures, and resident rights.
Serve as a resource and role model for dementia care best practices.

Build positive relationships with residents and their families.
Communicate regularly with family members regarding resident well-being and changes in condition.
Address concerns, questions, and service issues promptly and professionally.
Participate in family meetings and care plan reviews.

Maintain accurate and timely resident records and documentation.
Ensure compliance with all state licensing regulations, company policies, and community procedures.
Monitor incident reports, follow-up actions, and documentation requirements.
Assist with regulatory surveys, audits, and inspections.
Maintain confidentiality of resident information in accordance with HIPAA and company standards.

Promote a safe, clean, and therapeutic environment for residents.
Monitor quality of care and identify opportunities for improvement.
Assist in implementing programs that support resident engagement, dignity, and independence.
Support emergency preparedness and safety initiatives.

Pay: $24.00 - $26.00 per hour
