POSITION SUMMARY

Please note: This position is required to have a Bachelor’s degree.

Provide individual case management services to clients, including obtaining documents, employment seeking, housing assistance. This position is required to work out in the community for the majority of the time, providing transportation, network with employers, community resource providers and housing resources. This position works closely with the Veterans Administration for coordination of care tasks.

PRIMARY ACCOUNTABILITIES

Achieve Results

Connect client to community resources to meet basic needs, employment, educational, and housing needs.

Affect the improvement of clinical outcomes for each client. In collaboration with the Clinical Manager and/or Treatment Team, lead the recommendation and implementation of treatment plans for all clients as assigned. Ensure all services provided accurately and effectively meet individual client needs.

Ensure all client records are signed off by appropriate supervision and kept accurate and current. Each client record must reflect accurate information regarding all counseling services provided and subsequent record of outcomes and issues.

Actively engage with outside resources to support the clinical needs of each client. Develop and maintain a network of resources, which results in the efficient referral of clients for additional support.

Operational Excellence

Ensure all direct and indirect services provided to each client are delivered consistent with their programmatic intent and outcomes. Comply with all relevant laws, regulations, accreditation standards, and best practices in the day-to-day activities associated with delivering all care.

Ensure all client records, including progress reports, summary reports, and related documentation, are timely accurate, and uphold professional standards.

Support the development of the organization’s ongoing Quality Assurance initiatives. Provide information, resources, or expertise as requested, and ensure all services provided are compliant and consistent with such QA endeavors.

Relationship Management

Develop and maintain positive, effective relationships with all clients and their respective treatment teams. Work closely with other support staff to ensure that client needs are being met.

Maintain positive, effective working relationships with other staff members, volunteers, and referral sources critical to the efficient and effective care of each client.

Develop and ensure positive working relationships with regulators, state surveyors, accreditation resources, referral agencies, and related external partners essential to the success of the organization and the clients we serve.

Leadership and Workplace Culture

Uphold and ensure all activities and conduct are compliant with internal policies and relevant laws and regulations. Model the organization's values and mission at all times, and act with good stewardship of the organization’s resources.

PRIMARY TASKS & DUTIES

Case management, daily travel within the KC metro area

Client record updates and related client file maintenance, including treatment plans, individual case management notes in Community CareLink

Developing contacts and relationships with important referral resources within the community.

Ongoing collaboration and interaction with community partners and staff.

Additional duties as assigned, including case management support

Demonstrate exceptional interpersonal skills and the ability to communicate with a diverse and sometimes unwilling client population. Effectively manage conflict, promote change and growth, and inspire the development of all clients served.
Build consensus and focus within the Benilde Hall organization and among various community organizations.
Travel throughout the region as necessary

POSITION REQUIREMENTS

Experience

Demonstrated expertise and success providing relevant, case management and support to client groups of similar needs or requirements.
Demonstrated knowledge of trends and issues, laws and regulations related to the development and delivery of client care so noted.
Demonstrated success in establishing relationships with individuals and organizations of influence within the social services community, including civic leaders, charitable agencies, related government entities, fundraising sources, etc.
Demonstrated knowledge of federal, state and local community resources, key contacts, government entities, etc.

Education

Bachelor’s degree ,with emphasis preferably in counseling, psychology, rehabilitation or related field.
