Superior Hospice is looking for an experienced RN Case Manager to provide hospice care throughout Mankato and Southern Minnesota.
You'll take the nursing skills you've already developed and use them in an environment with greater independence, one-on-one patient care, and a very different pace than traditional bedside nursing.

You're Still Using Your Clinical Judgment
Hospice isn't simply sitting at the bedside.
A patient's breathing changes.
Pain is increasing despite the current medication plan.
Confusion or agitation suddenly develops.
A wound is worsening.
A caregiver reports that something just seems different.
You assess. You determine what's happening. You coordinate the response.
The difference is that your goal isn't always another procedure, test, or hospitalization.
Your goal is determining what will provide the greatest comfort, dignity, and quality of life for the patient.
What You'll Do
- Manage an assigned hospice patient caseload.
- Perform comprehensive nursing assessments.
- Develop and update individualized plans of care.
- Manage pain, dyspnea, agitation, nausea, wounds, medications, and other symptoms.
- Recognize changes in condition and determine appropriate next steps.
- Educate patients and caregivers about medications, symptoms, disease progression, and comfort measures.
- Coordinate with physicians and pharmacies.
- Collaborate with social workers, chaplains, hospice aides, facility staff, and other healthcare professionals.
- Support patients and families through changing clinical needs.
- Maintain accurate and timely clinical documentation.
- Help patients remain comfortable wherever they call home.
Your Acute-Care Experience Can Translate Extremely Well
We're especially interested in experienced RNs from:
- ICU / Critical Care.
- Emergency Nursing.
- Oncology.
- Med/Surg.
- Palliative Care.
- Home Health.
- Hospice.
- Skilled Nursing.
- Long-Term Care.

If your experience has taught you to assess thoroughly, recognize subtle changes, prioritize needs, communicate with providers, and remain calm when situations evolve, you already have skills that matter tremendously in hospice.
Hospice experience is preferred, but it isn't the only background we'll consider.
What We're Looking For
- Active Minnesota Registered Nurse (RN) license.
- Strong clinical nursing experience.
- Excellent assessment and critical-thinking skills.
- Confidence working independently.
- Ability to manage and prioritize a patient caseload.
- Strong patient and family communication.
- Excellent organization and documentation skills.
- Valid driver's license and reliable transportation.
- Willingness to travel throughout Mankato and surrounding Southern Minnesota communities.
