Position Summary
The Tradition LPN Supervisor is responsible for coordinating and delivering all aspects of nursing care to residents in both the Assisted Living and Memory Care settings. This role ensures residents’ physical and emotional well-being through skilled clinical care, medication administration, and ongoing health monitoring.

- Maintains professional, accurate, and timely communication with residents and their families, fostering active engagement and high levels of customer satisfaction.
- Effectively redirects and engages residents in a social and supportive manner, promoting a positive environment.
- Utilizes creative and personalized interventions to meet residents' social and clinical needs.
- Embraces the universal worker philosophy by assisting with all aspects of resident care and support necessary.
Role Specific
- Implement and coordinate resident care collaboratively with physicians and other healthcare professionals, ensuring timely communication and accurate documentation following regulatory and organizational standards.
- Provides care-related education and information to patients, CNAs, and families as appropriate.
- Interacts with other departments to ensure optimum patient care and teamwork.
- Supervises CNAs while on duty and provides training and coaching on all aspects of resident care.
- Takes ownership and responsibility for resident care, anticipates needs, and takes proactive measures for ALL residents on the unit, not just on assignment.
- Follows the physician’s order and medication administration policy as it applies to the safe medication and treatment administration, including completing administration on time.
- Appropriately documents medications, treatments, and other pertinent findings related to patient care within Sigma Care every shift to thoroughly and accurately reflect resident condition and care interventions.
- Notes and transcribes physician orders for medications and treatments accurately and appropriately.
- Gathers all pertinent information and completes incident reports appropriately.
- Provides a high standard of care by auditing and ensuring compliance as assigned.
- Addresses any scheduling conflicts or issues that arise, finding solutions to maintain required staffing levels and patient care standards.
- Carries and actively monitors assigned radio and work phone to ensure timely communication, effective team leadership, and real-time response to resident and operational needs.
- Utilizes all company-provided technology, including the work order system, email, Outlook calendar, and other designated software, to communicate with Facilities, Housekeeping, MorseLife Skilled Nursing facility, and outside providers, ensuring care coordination and support of daily operations.
- Maintains confidentiality under HIPAA and promotes resident dignity, rights, and autonomy.
- Performs all other duties as assigned.
What You’ll Bring
To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Associate’s degree or equivalent from a two-year college or technical school, with a minimum of 2 years of related experience in Memory Care; or an equivalent combination of education and experience.
- Strong written, verbal, and interpersonal communication skills.
- Experience with Electronic Medical Records (EMR) systems.
- Proficient in basic computer use and web-based software applications.

- Valid State of Florida LPN License
- Valid CPR Card
- Valid First Aid Certification
