Part-time: 3 days /wk. 8:00 a.m. – 4:00 p.m.
Essential Duties and Responsibilities:
Team leadership with a caring, positive approach. Computer skills and the ability to multitask are essential.
The Licensed Practical Nurse (LPN) is responsible for assisting the Team Leader in participant care and the day-to-day activities of the Sunshine Center Program. Act as Charge Nurse in the absence of the Adult Day Care Manager. Ensure positive communication, a clear understanding, and flow of activities, and collaboration with each team member and volunteer to ensure a positive experience for all participants and their families.
This includes assisting the Nurse Manager in her absence with staffing, directing, and implementing the program, supervising the activities and program aides, and the health care of participants enrolled in the Day Care program.
1. The LPN is responsible for collaborating with the Team Leader, Nurse Manager or Charge Nurse to ensure that each participant receives quality supervised person-centered care as stated in their individual care plans.
2. Is alert and attentive to participants’ needs in the areas of health, nutrition, and therapeutic programs in accordance with the provisions mandated by the State of Florida for Adult Day Care Centers.
3. Attend morning team meetings with fellow team members, including C.N.A.’s, and volunteers, to ensure a clear understanding of each team member’s assignments, good team communication and collaboration. Ensure that each participant receives the necessary quality assistance and supervision possible, as directed by the Team Leader and Adult Day Care Manager.
4. Assist each team member or volunteer as needed.
5. Observe each participant and immediately report any changes in their condition to the Adult Day Care Nurse Manager, or the charge Nurse on duty
6. Assist with monitoring and recording of routine vital signs and weights each month and PRN if signs of illness are noted.
7. Ensure each participant is receiving assistance as needed with personal care and incontinence care to avoid skin breakdown.
8. Dispense medications as prescribed by the participant’s physician in the absence of the Nurse Manager
9. Ensuring and maintaining clean bathrooms. and report to the Team Leader or Charge Nurse any maintenance of equipment required.
10. Ensure the use of infection control/prevention techniques in the rendering of care;
11. Assist with cutting up and serving daily diet orders efficiently to maintain food safety and temperature;
12. Assist with daily meal service to ensure participants receive meals prescribed by their attending physician; assist with feeding as needed.
13. Report inadequacies in dietary or fluid intake to the Team Leader and Adult Day Care Manager or Charge Nurse.
14. Assist in the provision of emergency care, as well as monthly fire drills.
15. Check participant clothing, incontinence and personal care supplies and obtain the same to ensure adequacy of amounts on hand.
16. Assisting with the implementation of the daily activity calendar and activities as assigned.
17. Act as a group leader or assistant for participants in the implementation of the above planned activities. Record the participant's daily activities on the appropriate activity sheet.
18. Assist with tidiness and cleanup of the facility and other duties requested by the Team Leader, Nurse Manager and/or Charge Nurse as needed.
19. Ability to maintain regular, punctual attendance consistent with the organization's policies.
QUALIFICATIONS:
- Must pass a Level II Background Screening through the ACHA Clearinghouse (https://info.flclearinghouse.com/)
- High school diploma or GED.
- Have a current Certificate of C.P.R. and First Aid training.
- Have passed the Florida Board of Nursing Exam and hold a current certificate as a Licensed Practical Nurse in good standing on the Florida Nurse Registry with at least one year of experience in geriatric care in an acute, long-term, or home care setting.
- Provide a written physician statement within 45 days of employment to be in good health, including freedom from communicable disease, including tuberculosis in the communicable form.
- Must be knowledgeable and sensitive to the needs of the elderly, those affected by cognitive impairment, and their caregivers.
- Must have strong attention to detail, work well under pressure, and meet deadlines while maintaining a positive attitude.
- Ability to maintain confidentiality of client information.
- Ability to work alone on own initiative, as well as work collaboratively with a small team.
- Able to effectively work with a diverse population and bring the best out in others;
- Excellent verbal and written communication skills, including the ability to communicate effectively with internal and external customers.
- Must be able to work under pressure in a fast-paced environment and meet deadlines while maintaining a positive attitude.
- Ability to work independently, prioritize, and to carry out assignments to completion within the supervisor’s instruction parameters, recommended procedures and accepted practices.
- Affinity/empathy for older adults is a must.
- Works well with others, has strong customer service skills, and interpersonal skills.
- Is able to maintain a courteous, friendly, and positive attitude while working in a fast-paced environment with a diverse population.
- Is able to quickly resolve customer service issues with positive outcomes.
- Has experience working with those afflicted with brain disorders, including Alzheimer’s disease and Other related dementias, and their families.
- Has computer experience in Word; Excel a plus;
- Has experience speaking to groups.
PHYSICAL ACTIVITY REQUIRED
- Must be physically able to stoop, kneel, crawl, bend both knees, turn, reach, and extend arms in any direction.
- Must be able to assist with transferring the participants to and from wheelchairs, chairs, and toilets.
- Grasping: Must be able to lift and carry up to 50 lbs.
- Ability to stand for long periods of time.
- Talking: Must be able to express or exchange ideas by means of the spoken word.
- Hearing: Must have the ability to hear alarms, listen to the participant’s breathing, and receive detailed information through oral communication.
- Sitting: Ability to sit for prolonged periods.
- Ability to safely and successfully perform the essential job functions consistent with meeting qualitative and/or measurable productivity standards.
