Full Job Description

Clinical Pharmacist in the Population Health and Prescription Management (PHARXM) provides clinical, and educational functions as necessary to support the overall patient care provided by PHARxM. The Clinical Pharmacist provides medication therapy management for patients as defined under medical staff approved protocols and contributes to process improvement for the clinic and department.

Professional Duties

Follow medical staff approved protocols in management of patients defined under PHARxM department.
Provide comprehensive medication therapy consults to patients referred to PHARxM.
Develop individualized medication plans to mitigate the risk of adverse side-effects.
Provide general drug information to Providers to support the delivery of quality patient care.
Document patient interactions and consultations in the patient's electronic medical/health record (EMR/EHR) in accordance with established departmental policies, procedures, and timeframes.
Refer patients to other departments and/or programs for untreated health problems.
Conduct comprehensive medication histories, assess patient compliance, and recommend therapy modifications to optimize clinical outcomes.
Assist in transitions of care by ensuring continuity, accuracy, and safety of medication therapy across different care settings.
Ensure safe, effective, and cost-efficient medication therapy for all patients.
Establish and maintain policies and procedures for drug usage, distribution, and control to ensure regulatory compliance and safe medication practices.
Perform other duties as assigned.
Serve as an ambassador for Desert Oasis Healthcare and Family Hospice Care at all times and positively shape the customer experience

Qualifications

PharmD, BS in Pharmacy, or foreign equivalent, with a valid CA RPh license required.
Current California Pharmacist License required.
0-2 years of pharmacy experience required.
Active member of a State or National Pharmacy Organization, preferred.
Ability to utilize Microsoft Office applications (Word, Excel, Outlook, Power Point) and other computer software/applications.
Ability to effectively communicate with individuals within all levels of the organization, patients, patient family members, providers, vendors and others.
Ability to work independently and as part of a team.
Ability to manage time effectively and prioritize tasks to meet established deadlines.
BLS required within 60 day of hire. BLS not required for employees who are not involved in direct patient care.
At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.

Walking: Approximately 13% of day.
Lifting: 0 - 20 lbs. approximately 40% of day.
Bending: Approximately 10% of day. Kneeling approximately < 10% of day.
Hearing/Visual Acuity: Adequate for use with computers, telephone/mobile device and other office equipment approximately 50% of day.
Computer: Highly technical work environment. Must be able to work 6+ hours per day using computers, telephone/mobile device and other office equipment.
Reaching: Above head 75 degrees approximately 10%.
Hand grip dexterity: Approximately 60% of day.
