Full Job Description

Summary

Angelic Home Health Care, Inc. is seeking an organized, dependable, and detail-oriented Intake Coordinator to support the referral and admission process for our patients.

This position plays an important role in coordinating new patient referrals, reviewing documentation, communicating with patients and healthcare providers, assisting with scheduling and staffing, and ensuring information is processed accurately and on time.

The ideal candidate is comfortable working in a fast-paced healthcare environment, can manage multiple priorities, pays close attention to detail, and follows through on tasks from beginning to completion.

Key Responsibilities

Receive and process new patient referrals from hospitals, physicians, facilities, and other referral sources.
Review referral information and documentation for accuracy and completeness.
Assist with verifying insurance eligibility, coverage, and authorizations when needed.
Communicate with patients, families, physicians, hospitals, and other healthcare providers.
Coordinate and assist with scheduling and staffing patient visits, including nursing, therapy, and other ordered services.
Follow up on missing documentation, physician orders, DME, and other items needed for patient care.
Enter and maintain accurate patient information in the agency's electronic system.
Keep track of pending referrals and follow up to ensure tasks are completed on time.
Communicate updates and important information to the appropriate office and clinical staff.
Work closely with clinical managers, billing, scheduling, and other departments.
Provide professional and friendly customer service to patients and referral sources.
Maintain patient confidentiality and follow HIPAA requirements.
Perform additional administrative and coordination duties as needed.

Qualifications

Previous healthcare office, medical office, administrative, intake, scheduling, staffing, or customer service experience preferred.
Home health experience is a plus but not required for the right candidate.
Strong attention to detail and ability to maintain accuracy.
Strong organizational and time-management skills.
Ability to manage multiple tasks and priorities in a busy environment.
Ability to meet deadlines and handle time-sensitive responsibilities.
Strong follow-up skills and ability to take ownership of assigned tasks.
Comfortable communicating professionally by phone, email, and in person.
Good computer skills, including email, Microsoft Office, scanning, and document management.
Experience with EMR or healthcare software is preferred; Healthcare Synergy experience is a plus.
Basic understanding of scheduling, numbers, and visit frequencies is helpful.
Ability and willingness to learn home health processes, documentation requirements, and regulations.
Bilingual, Spanish is a plus.
Dependable, professional, and able to work well as part of a team.

What We're Looking For

We are looking for someone who is organized, attentive, responsible, and willing to learn. This is a high-volume position where details and deadlines matter. The right candidate should be able to learn from feedback, avoid repeated errors, communicate when they need assistance, and consistently follow through on their responsibilities.

Schedule & PayHours:

Monday–Friday, 8:30 AM to 4:30 PM
Weekends/Holidays: Off
Pay Period: Bi-weekly
Pay Rate: $20 – $26 per hour, based on experience
