Job description

As a Coordinated Family Support (CFS) Specialist at FMRC Health Group, you'll play a crucial role in assisting individuals and families by coordinating various services and supports that enable adults to maintain their residence within their family home. Your daily tasks will include:

Responsibilities:

Responsibilities
Provide guidance and support in accessing generic and community resources such as SSI, Medi-Cal, IHSS, and Cal-Fresh, demonstrating strong knowledge of available services within the San Fernando Valley (SFV) and Antelope Valley (AV) areas.
Assess each individual’s and family’s ability to obtain, utilize, and maintain these resources, including navigating government systems like Social Security, completing applications, and renewing benefits.
Coordinate and manage scheduling for medical, educational, vocational, and personal appointments as needed.
Identify and arrange transportation options, including ACCESS Services and other community-based programs, to support client mobility and engagement.
Assist with future planning initiatives to promote long-term stability, independence, and quality of life.
Facilitate daily living activities and community participation opportunities that encourage personal growth, independence, and social engagement.
Maintain accurate progress notes, documentation, and reports in compliance with program and Regional Center requirements.
Support clients in developing practical skills for budgeting, social interactions, and community involvement.
Connect individuals to inclusive programs, social events, and volunteer or educational opportunities that promote meaningful participation.
Collaborate with service providers such as ILS, SLS, Day Programs, and Employment Services to ensure cohesive and coordinated care.
Develop and implement Plans of Action (POA) to address identified needs, set measurable goals, and monitor ongoing progress.

Qualifications

Education & Experience: Associate’s or Bachelor’s degree in Human Services, Social Work, Psychology, or a related field, plus at least one (1) year of experience working with adults with developmental, cognitive, or physical disabilities; or a minimum of three (3) years of case management experience serving adults with disabilities.
Case Management: Proven experience providing case management services and coordinating supports for individuals with developmental or intellectual disabilities.
Communication: Excellent written and verbal communication skills, with the ability to build rapport and maintain strong, professional relationships with clients, families, and community partners.
Knowledge: In-depth understanding of community and generic resources, including SSI, Medi-Cal, IHSS, Cal-Fresh, and related public benefit systems.
Professional Skills: Ability to work independently, manage multiple priorities, demonstrate sound judgment, and remain calm and effective in challenging or crisis situations.
Preferred Experience: Familiarity with mental health services, developmental disability systems, and regional center service coordination.
Transportation: Must possess a reliable vehicle, valid driver’s license, and current car insurance.
Compliance: Must successfully complete a criminal background check prior to employment.
Language: Fluency in Spanish (reading, writing, and speaking) is required.
Flexibility: Willingness and ability to travel and attend client appointments throughout the San Fernando Valley (SFV) and Antelope Valley (AV) areas.
