Job Summary: The Anthem Hospice RN Case Manager is that member of the interdisciplinary team who is the pivotal person in identifying the physical, psychosocial, behavioral health, and spiritual needs of the patient and family. He or She initiates appropriate intervention and support for the patient and family upon admission to Anthem Hospice and provides a continuously appropriate, comprehensive, and responsive plan of care.

Job Type: Full time or part time

Requirements and Qualifications:

Current RN licensed to practice in the state of California
Minimum of 1 year prior experience in hospice
Proficient nursing skills aligned with accepted standards of practice
Basic understanding of the interpersonal dynamics operating within the working relationships of an interdisciplinary health care team, and knowledge of the process of dying and bereavement
Reliable transportation with appropriate license and insurance coverage for driver and passengers.
Experience with HospiceMD preferred
Bachelor's degree preferred
