Full Job Description

Job Title: Case Manager

Reports To: Executive Director
Location: Northwest Florida Region; Office is in Panama City, FL
FLSA Status: Non-Exempt / Full-Time

Position Summary:

The Case Manager works to engage and assist individuals and families experiencing homelessness or at risk of homelessness. This position identifies those in need through coordinated entry and street outreach. Once engaged, the Case Manager helps individuals connect to critical resources including housing, shelter, mental health and substance use treatment, and other supportive services. This position plays a vital role in transitioning participants from homelessness to housing stability.

Essential Duties and Responsibilities:

Conduct comprehensive assessments to determine client needs, eligibility for services, and barriers to housing.
Develop individualized service plans with measurable goals focused on housing stability and increased self-sufficiency.
Connect clients to mainstream benefits, mental health/substance use resources, employment programs, legal aid, and housing programs.
Provide ongoing support, advocacy, and follow-up to ensure continuity of care and progress toward client goals.
Assist clients in gathering necessary documentation (e.g., ID, Social Security card, income verification) required for housing placement.
Monitor housing placements and provide aftercare support to help prevent returns to homelessness.
Conduct street outreach to locate, engage, and build rapport with unsheltered homeless individuals and families.
Maintain safety protocols and confidentiality
Collaborate with law enforcement, healthcare providers, local agencies, and community partners to identify and assist homeless populations.
Maintain accurate and timely documentation of outreach encounters in the Homeless Management Information System (HMIS).

Required Qualifications:

Bachelor’s degree in social work, human services, psychology, or related field preferred; equivalent experience considered.
At least 3 years of experience working with vulnerable populations (e.g., homeless, mentally ill, substance use disorder, veterans, etc.).
Strong interpersonal and communication skills with an ability to build trust with diverse clients.
Knowledge of community resources, entitlement programs, and housing systems.
Proficiency in case documentation and basic computer use; experience with HMIS preferred.
Valid driver’s license, reliable transportation, and ability to work in field-based settings.
Ability to work flexible hours including occasional evenings or weekends.

Preferred Qualifications:

Experience using trauma-informed care, motivational interviewing, and/or housing first approaches.
Bilingual skills (especially Spanish).
Familiarity with local Continuum of Care (CoC) processes and Coordinated Entry systems.
Experience using database management systems.

Working Conditions:

Occasional travel to various locations, including streets, encampments, and shelters.
May involve exposure to various weather conditions.
Must be comfortable working independently and in non-traditional environments.

Compensation and Benefits:

Salary Range: $17.00 to $19.00 per hour
Benefits include health, dental and vision insurance, paid time off, mileage reimbursement, and training opportunities.
