Full Job Description

Are you passionate about helping children and families navigate behavioral health services? We're seeking compassionate, experienced clinicians to join our team as a Pediatric Behavioral Health Care Manager. In this role, you'll partner with members, families, providers, schools, and community organizations to coordinate care, remove barriers to treatment, and improve long-term health outcomes through a collaborative, whole-person approach.

Position Highlights

Start Date: August 24
Schedule: Monday–Friday, 8:00 AM–5:00 PM EST
Travel: 10–40% local travel within a 60-mile radius to member homes, provider offices, schools, and community settings
Must be located in: Miami-Dade County or Monroe County

What You'll Do

Complete comprehensive member assessments to identify care management needs and develop individualized care plans.
Coordinate care with members, caregivers, physicians, behavioral health providers, schools, and community resources.
Conduct telephonic, virtual, face-to-face, and home visits as needed.
Monitor member progress, evaluate care plan effectiveness, and adjust interventions to support positive outcomes.
Maintain an ongoing caseload while providing regular outreach, education, and care coordination.
Identify barriers to care and connect members with behavioral health, long-term services and supports (LTSS), and community resources.
Collaborate with interdisciplinary care teams (ICT) to ensure continuity of care.
Utilize motivational interviewing techniques to educate, support, and empower members throughout their healthcare journey.
Document assessments, interventions, referrals, and member progress in accordance with organizational and regulatory guidelines.

Qualifications

Active Florida license in one of the following:
LCSW
LMHC
LMFT
LMSW
APRN
RN
At least 2 years of experience in healthcare, preferably in care management, behavioral health, or a related clinical setting.
Valid driver's license, reliable transportation, and auto insurance for local travel.
Working knowledge of EMR systems and HIPAA regulations.
Familiarity with community resources and care coordination.
Excellent communication, organization, critical thinking, and time management skills.
Ability to work independently while collaborating effectively with interdisciplinary teams.
Proficiency with Microsoft Office and electronic documentation systems.

Preferred Qualifications

Certified Case Manager (CCM)
Experience working with pediatric populations and behavioral health services.

Pay: $38.00 - $40.00 per hour
