Full Job Description

Job Overview:

This position is responsible for ensuring that program services remain high-quality, client-centered, trauma-informed and survivor-led. The Program Specialist will be responsible for being responsive and timely with the delivery of the following core services:

Conducting initial screenings, intakes and housing supports assessments
Partners with participants to develop individual program plans
Responsible for accurate and up-to-date case note documentation for all services
Actively engages in professional development activities; maintains current knowledge on prevention and intervention strategies; trauma-informed practices; and utilizes best practice models as a guideline for case management, outreach and community engagement
Assists with transportation to allow clients to access services and activities in the community, that contribute positively to their overall well-being
Maintains confidentiality of all written/ electronic materials relating to participant cases
Provides ongoing communication with the Director of Programs
Monitors and tactfully clarifies matters regarding case file documentation, partner referrals, intakes, discharges, and other concerns
Identifies service gaps and opportunities for program development and expansion.
Required to work a flexible schedule to facilitate program
Inclusive thought that demonstrates respect and sensitivity for cultural differences. Ability to make culturally appropriate recommendations to staff and clients, and demonstrated experience working with diverse populations
Identifies and resolves problems in a timely manner; gathers and develops alternative solutions; works well in group problem solving situations
Approaches others in a tactful manner; treats others with respect and consideration regardless of their status or position; maintains healthy communication and proper boundaries
Uses professional judgment when interacting with clients, staff and partners of CHH. Understands how language is important in shaping client and case management interactions
Must have basic computer abilities including Microsoft Office (Word, Excel, Outlook and PowerPoint) and a willingness to learn other programs maintained by CHH
Ability to work independently and synergistically as part of a team
Provide on call coverage and cover shift as needed
Excellent oral and written communication
Other related duties as assigned
Experience in case management and/or providing direct services to vulnerable populations (homeless, former foster youth, domestic violence clients and/or survivors of trauma) is preferred.

Education and Qualifications:

Bachelor's degree human services, such as, but not limited to Social Work, Psychology, Sociology, Early Childhood Development, or a related field.
Must pass background checks to include:
DHHS Child/Adult Abuse and Neglect Central Registry
State Patrol Central Registry of Sex Offenders
DMV Check
Criminal records check
Must possess an acceptable driving record and current driver’s license
Knowledge and understanding of trauma informed care preferred
Must present as a constant professional role model to the participants and staff in actions, language, dress and general attitude
Must have understanding, patience, empathy kindness and a concern for participants
