Full Job Description

We are seeking a Residency-trained Clinical Pharmacist

JOBSITE: Academic Medical Center in Pacific States

$80–$116/hour
$10,000 Sign-On Bonus
$7,500-$10,000 Relocation Assistance
Permanent Full-Time Position
NO Travel | One Hospital
Day Shift

Join a respected 400+ bed acute care hospital / Academic Medical Center

YOUR ROLE:

Interdisciplinary patient rounds
Work decentralized as a Clinical Pharmacist
Verify and review physician medication orders
Evaluate medication therapy for safety, efficacy, allergies, interactions, and appropriate dosing
Perform pharmacokinetic dosing and therapeutic drug monitoring
Manage anticoagulation therapy
Complete renal dose adjustments and IV-to-PO conversions
Compound and accurately dispense medications
Provide drug information and clinical recommendations to physicians, nurses, and patients
Support formulary management and therapeutic interchange initiatives
Document clinical interventions and medication safety activities
Promote safe, evidence-based medication practices throughout the hospital

QUALIFICATIONS:

Doctor of Pharmacy (PharmD)
PGY1 Residency preferred, or at least one year of acute care hospital pharmacy experience
