Full Job Description

About Us:

Columba Services Inc. is a nonprofit organization dedicated to helping individuals who have struggled with homelessness. Many of our residents have experienced significant trauma in their lives and are struggling with mental illness and/or substance use disorder. We provide a safe and stable environment for all the individuals we serve while treating them with Respect and Dignity. A Harm Reduction approach is used at Columba Services.

Essential Functions:

Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

Provide trauma-informed, case management services to individuals who are struggling with substance use disorder (SUD)and/or mental illness (MI).
Create person-centered goals for service planning with individuals.
Assist individuals with housing stability, budgeting, health and wellness.
Work with individuals to break through barriers in attaining goals assist with advocating for themselves and successfully moving toward self-sufficiency.
Connect individuals to resources for medical, mental health, substance use, vocational, and other resources.
Participate in the intake and admission process for individuals.
Co-facilitate groups around harm reduction, daily living skills, health relationships, budgeting and others.
Immediately reports incidents or sensitive situations to supervisors.
Update community referral sources for individuals.
Maintain and expand relationships with community providers.
Actively participate in staff and community meetings.
Perform other related duties as assigned.

Qualifications and Requirements:

Bachelor’s degree in Social Work or similar field
At least two years of experience working with people who struggle with addiction and/or homelessness
Ability to work with individuals and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences.
Ability to communicate effectively verbally and in writing.
Must have good problem-solving skills.
Proficient computer skills with MS Office (Word, Excel and Outlook)

Physical Demands:

While performing the duties of this job, the employee is regularly required to stand, walk, sit and/or climb stairs. Occasionally, the employee will have to lift and/or move up to 10 pounds.
