Full Job Description

A licensed home care agency where the hardest cases get a real owner, not a place in the queue.

People Care has served New York patients and families for 50 years. We employ roughly 4,300 caregivers across New York City and Long Island. This seat owns the cases that are actively at risk of breaking — and works them until they are stable enough to hand back.

WHAT YOU OWN

The escalated and at-risk case queue. The cases where continuity of care is already in question — owned end to end until they are resolved and stable again.
The cross-functional handoff. You are the point of contact when a case needs more than a scheduling fix: authorization disputes, eligibility questions for Clinical, or a workplace-relations matter driving a coverage gap.
Complex matching. Partnering with Coordinators on difficult client/aide matches — language, geography, clinical need, prior disruption — and stepping in directly where continuity has already broken once.
Audit-ready documentation. Every escalated file documented to program and managed-care standard.
Closing the loop. Every escalation outcome recorded in the case record and handed back to the assigned Coordinator, so the fix sticks.

WHO YOU ARE

You are someone who cares deeply about patients and the caregivers who serve them. You are transparent — you surface a problem early rather than hoping it resolves itself. You go beyond what the task asks when a client's care depends on it. You row together with clinical, compliance and billing rather than working around them.

2+ years of home care coordination, case management, or care management experience.
HHAeXchange fluency, or a comparable scheduling / EVV platform.
Comfort with ambiguity. You can pick up a half-resolved case with no script and work it to closure.
Clear, calm communication with clients, families, aides and internal partners — including the difficult conversations.
Comfort in a structured operating system (we run on EOS)

NICE TO HAVE

Prior experience in a lead, senior coordinator, or case-management-supervisor capacity.
Experience with DFTA / NYC Aging, NHTD, CDPAP, or managed long-term care
Bilingual in any language spoken by our client population.

People Care has served New York patients and families for 50 years. Most of Coordination's work holds a stable case list steady. This seat exists for the cases that do not hold on their own — so an escalation gets real ownership instead of competing with forty other stable cases on one Coordinator's list. Every case that breaks continuity is a patient who may go a day without coverage.

WHAT YOU GET

Office & Schedule

100% on-site, 40 hours per week, at our Brooklyn office at 1649 61st Street. This work depends on being in the room with the coordination team.

Pay: $80,000.00 - $85,000.00 per year
