The Facilitator serves as the care coordinator and primary guide for the Wraparound process. This role leads, coordinates, and monitors the individualized Plan of Care to ensure each phase reflects the family’s voice, culture, strengths, and goals. The Facilitator promotes high-fidelity Wraparound by ensuring that Engagement, Plan Development, Implementation, and Transition follow best practices and uphold the ten Wraparound principles. As a key member of the Child and Family Team (CFT), the Facilitator gathers information throughout the CFT process to support a written, individualized plan based on the youth and family’s identified strengths and needs. The Facilitator organizes and leads youth and family meetings, coordinates services across systems including child welfare, mental health, juvenile justice, education, and tribal systems when applicable and maintains cohesive team engagement. The Facilitator is available to youth and families for crisis response and helps unify team efforts to provide coordinated, high-fidelity care. Work schedules may include evenings, weekends, and holidays based on program needs. Staff participate in crisis response as part of their regular duties, and all emergency responses are fully compensated. The Facilitator also collaborates with community, school district, and county agencies as needed. experience with emotionally disabled children or youth in an inpatient or outpatient healthcare setting (internship/practicum hours may be substituted).
- One (1) year case management experience preferred.
Bachelor's Degree in social services or related field required.
