Position Overview
As a member of the Reentry team, the BSCC Case Manager provides comprehensive case management services to individuals transitioning from incarceration back into the community, ensuring successful reintegration and compliance with parole conditions. This position involves conducting initial assessments, developing individualized reentry plans, and coordinating access to essential services including housing, healthcare, mental health treatment, substance abuse programs, and educational opportunities. The case manager maintains regular contact with parolees, monitors progress toward goals, facilitates connections with community resources, and collaborates with parole officers and other stakeholders to support positive outcomes and reduce recidivism risk. This position will be filled contingent upon a criminal background clearance, possession of a valid driver’s license and automobile insurance, educational transcript and certifications.
Roles & Responsibilities
Primary Responsibilities
- Conduct initial intake assessments and develop individualized case plans for assigned participants
- Maintain regular contact with participants through scheduled office visits, field visits, and community-based meetings to monitor compliance with parole conditions
- Document all interactions, observations, and progress in case management system (Apricot) and maintain accurate, timely case records in accordance with prescriptive KPI’s
- Monitor adherence to all court-ordered and parole board conditions including curfews, geographic restrictions, substance abuse treatment, community service, and restitution payments Client Support & Case Management
- Maintain an established caseload and provide comprehensive reentry case management services
- Monitor participant progress, compliance with supervision requirements, and goal attainment
- Coordinate referrals and ensure participant engagement with housing, employment, behavioral health, and other supportive services
- Practice Motivational Interviewing as the standard for client engagement
- Support data tracking, reporting accuracy, and program evaluation efforts
Safety and Professional Responsibilities
- Follow all safety protocols and report any public safety concerns immediately
- Identify potential public safety concerns and implement appropriate interventions or referrals
- Maintain and model professional boundaries and ethical standards
- Ensure confidentiality and compliance with all applicable laws and regulations
- Participate in ongoing training and professional development
Other Duties
- Participate in staff meetings, trainings, and organizational initiatives
- Support onboarding and training of new staff as needed
- Other duties as assigned
Resource Coordination and Support
- Connect participants with community resources including housing assistance, employment services, mental health treatment, substance abuse programs, educational opportunities, and social services
- Maintain relationships with service providers and advocate for participant’s needs while coordinating care across multiple agencies
- Follow up on referrals to ensure service engagement and effectiveness
- Offer ongoing support and counseling to help participants navigate challenges and maintain stability
Administrative and Legal Duties
- Maintain detailed records of all contacts, services provided, and progress toward reentry goals
- Generate statistical reports and data for program evaluation and compliance with state and federal requirements
- Attend court hearings, parole board meetings, and other legal proceedings to provide testimony and case updates as needed and approved by supervisor
- Collaborate with attorneys, judges, probation officers, and other criminal justice professionals
- Prepare pre-sentence investigation reports and violation reports as requested
- Work closely with law enforcement agencies, treatment providers, employers, family members, and community organizations to support successful reintegration
- Participate in multidisciplinary team meetings and case conferences to support departmental strategies
Specialized Functions
- Support educational goals including GED completion, vocational training, or higher education enrollment
- Facilitate family reunification when appropriate and help address relationship conflicts or dynamics that may impact successful reentry
- Connect participants with community-based support groups, mentoring programs, and volunteer opportunities to build positive social networks

Essential Requirements
- Current driver's license, valid auto insurance, and reliable transportation
- Successful completion of a comprehensive background check, as verified by law enforcement administration or other designated agency
- Provide educational transcript
- Bachelor's degree in Social Work, Psychology, Criminal Justice, or related field preferred
- Minimum of 2 years experience in workforce development or related field preferred
- Proficiency in Google Suite, Microsoft Office Suite, and database management systems
- Ability to work flexible hours, including occasional evenings and weekends
- Fluency in English (bilingual English/Spanish highly desirable)
