Job Summary
Lighthouse Care is seeking a compassionate, detail-oriented Nutrition Assistant to support the SOMOS Social Care Network (SCN) program under New York State's 1115 Medicaid Waiver. Working closely with the Registered Dietitian/Nutritionist and the Enhanced Social Services team, the Nutrition Assistant will coordinate nutrition-related services for Medicaid members experiencing Health-Related Social Needs (HRSNs).

The Nutrition Assistant will assist with medically tailored meal services, nutrition referrals, appointment scheduling, meal delivery verification, client outreach, and ongoing follow-up. This role ensures members receive timely nutrition interventions while maintaining compliance with SCN documentation, billing, and quality standards. The position is critical in improving food security, chronic disease management, and overall health outcomes for eligible Medicaid members.

Essential Duties & Responsibilities

Nutrition Navigation & Member Support

Coordinate nutrition services for Medicaid members referred through the SOMOS Social Care Network (SCN).
Contact eligible members to explain available nutrition services and assist with enrollment.
Schedule nutrition assessments and follow-up appointments with the Registered Dietitian/Nutritionist.
Conduct required 7-day, 30-day, and ongoing follow-up calls to assess member progress and satisfaction.
Educate members on healthy eating, medically tailored meal services, and available community food resources.
Maintain regular communication with members throughout their participation in the nutrition program.

Medically Tailored Meal Coordination

Coordinate and verify medically tailored meal deliveries.
Monitor meal delivery status through vendors and member outreach.
Track missed deliveries and coordinate timely resolution.
Document meal delivery verification for SCN billing purposes.
Escalate unresolved meal delivery issues to the Program Manager and Nutritionist.

SCN Care Coordination

Collaborate with SCN Screeners, Navigators, Community Health Workers, Care Managers, and healthcare providers.
Coordinate nutrition referrals generated through SCN screenings and Health-Related Social Needs (HRSN) assessments.
Ensure members receive timely nutrition interventions consistent with their individualized Social Care Plan.
Participate in interdisciplinary case conferences as needed.

Documentation & Compliance

Complete all documentation required by SOMOS SCN and Lighthouse Care.
Enter all completed billable nutrition activities into designated documentation systems.
Maintain accurate records supporting Medicaid reimbursement.
Ensure compliance with HIPAA, Medicaid regulations, and SCN program requirements.
Complete documentation within 24 hours of service delivery.

Minimum Qualifications

Education

Associate degree in Nutrition, Public Health, Human Services, Health Education, or related field required.
Bachelor's degree preferred.

Experience

Minimum three (3) years of experience in community health, patient navigation, case management, or care coordination.
Experience working with Medicaid populations preferred.
Experience supporting nutrition services, food insecurity initiatives, medically tailored meals, or social care programs preferred.
Familiarity with New York State Social Care Network (SCN), SOMOS, Health-Related Social Needs (HRSNs), or value-based care models is highly desirable.

Compensation & Benefits

Compensation: $7.00–$8.75 per completed billable unit, based on experience, productivity, and program requirements.
Billable units include verified meal deliveries, nutrition referrals, member outreach, appointment coordination, nutrition follow-up activities, and completed documentation in accordance with SOMOS Social Care Network (SCN) billing guidelines and Lighthouse Care policies.
All billable services must be completed, documented, and submitted within required timelines to qualify for reimbursement.
Ongoing training on SCN workflows, Medicaid documentation, and Enhanced Services.
