Virtual Registered Nurse – Admission/Discharge

Job Details:

Responsibilities

Facilitate seamless, timely and patient-centered admissions and discharges.
Complete required assessments and admission/discharge documentation.
Provide individualized education and support to patients and families.
Assess, plan, implement and evaluate nursing care using sound clinical knowledge and judgment.
Collaborate with the interdisciplinary team to coordinate care and support safe transitions.
Delegate appropriate patient care activities while maintaining responsibility and accountability for nursing care.
Respond appropriately to individual patient needs, including age, language, culture, health literacy and developmental or physical needs.
Maintain patient safety, infection prevention practices and established standards of care.
Maintain professional competency and participate in continuing education relevant to the clinical area.
Participate in unit-based performance improvement initiatives and support departmental goals.

Qualifications

Bachelor of Science in Nursing (BSN) required; candidates with an Associate Degree in Nursing (ASN) who are actively enrolled in a BSN program will also be considered.
Current New York State Registered Professional Nurse license required.
Current BLS certification from the American Heart Association required.
Previous patient care experience in a hospital setting preferred.
Strong communication, clinical assessment, prioritization and patient education skills.
Proficiency with electronic medical records (EMR, Epic preferred) and computer systems.

Posted Salary Range: USD $56.39 - USD $73.66 /Hr. :

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
