About Square Medical Group (SMG)
Square Medical Group’s (“Square” or “SMG”) mission is to deliver enduring, material improvements in both clinical and wellness outcomes to patients addressing co-occurring challenges with substance use disorder (SUD) and behavioral health (BH). We do this through delivering a specialized continuum of outpatient care of 1,000+ units/week to a population of ~5,000 Members across Massachusetts: medication management, in-person and virtual therapy for individuals, couples, and families, in-person and virtual intensive outpatient and partial hospitalization programs (IOP/PHP), transcranial magnetic stimulation (TMS), in-house laboratory services, as well as residential services.

Program Overview:

The residential recovery programs are structured rehabilitation-oriented treatment focused on trauma-informed care and harm reduction. It is designed for motivated individuals who want to enhance the quality of their day-to-day lives. Through active engagement in groups and the development of the community, we will work together to overcome challenges that interfere with our abilities to live life to the fullest.
Members will be assigned a weekly life skill to ensure ongoing cleanliness/maintenance of the house. Members are expected to take part in assigned life skills. Staff will help if Members are unsure about how to complete a task. Life Skills are necessary to independent living.
On average, Members spend between three to six months in the three phases of the residential program. Each phase of the program rewards Members with increasing levels of responsibilities and freedoms. Throughout the program, Members are supported by deep expertise from SMG’s outpatient services, our core capability. Members are also able to stay with our outpatient practice post-discharge for continuity of care.
Role Summary
The Clinical Screener serves as the clinical gatekeeper for admissions into Square Medical Group's Co-Occurring Enhanced Residential Rehabilitation Services (COERRS). This role is responsible for conducting comprehensive clinical assessments to determine whether referred patients meet medical necessity, ASAM criteria, and COERRS admission standards.
The Clinical Screener reviews referral documentation, conducts detailed screening assessments with patients and referral sources, evaluates psychiatric, medical, substance use, and psychosocial needs, and determines whether patients can be safely and appropriately treated within an ASAM Level 3.1 residential setting.
Working collaboratively with Patient Access, the Eligibility & Authorization Specialist, Program Directors, Clinical Directors, Medical Providers, and community referral partners, the Clinical Screener ensures timely, clinically appropriate admissions while maintaining patient safety and regulatory compliance throughout the admission process.
The Clinical Screener maintains clinical ownership of each referral from the time it is administratively cleared until the patient is admitted, denied, or removed from the waitlist.
*While this role will be primarily remote, some on site work will be required, so candidates MUST be local to the New England Area*
Key Responsibilities
Clinical Assessment & Admission Determination
- Review all clinical referral documentation, including biopsychosocial assessments, psychiatric evaluations, discharge summaries, medication lists, and supporting records.
- Conduct comprehensive clinical screening assessments with patients and referral sources.
- Evaluate each patient using ASAM Criteria and COERRS admission guidelines.
- Assess:
- Substance use and mental health history and severity
- Previous treatment episodes
- Overdose history
- Medication Assisted Treatment (MAT)
- Psychiatric diagnoses and current stability
- Suicide and violence risk
- Trauma history
- Medical conditions and medication needs
- Functional status and activities of daily living
- Housing status
- Legal involvement
- Recovery support systems
- Motivation for treatment
- Identify patients requiring additional medical, psychiatric, or clinical review before admission.
- Determine whether patients are clinically appropriate for admission into COERRS.
Clinical Documentation
- Complete all screening documentation within InSync following each assessment.
- Upload referral packets and supporting clinical documentation into the EMR.
- Clearly document admission recommendations and clinical rationale.
- Complete denial documentation and maintain denial tracking logs when appropriate.
- Ensure documentation meets regulatory, payer, and organizational standards.
Waitlist Clinical Management
- Maintain ongoing clinical oversight of patients on the active waitlist.
- Review updated documentation received from referral sources.
- Reassess patients when significant clinical changes occur.
- Confirm continued clinical appropriateness prior to admission.
- Communicate changes in patient status to residential leadership and the Eligibility & Authorization Specialist.
Admission Coordination
- Communicate admission decisions promptly to referral sources and internal teams.
- Collaborate with Program Directors and Clinical Directors regarding bed availability and clinically appropriate admissions.
- Coordinate admission timing with the Eligibility & Authorization Specialist to ensure patients are both clinically approved and financially cleared prior to admission.
- Serve as the clinical resource for admission-related questions.

Qualifications
Required
- Bachelor's degree in Social Work, Psychology, Counseling, Human Services, Nursing, or a related healthcare field.
- Minimum of two years of experience in behavioral health, substance use treatment, residential services, crisis services, case management, or healthcare admissions.
- Experience assessing individuals with co-occurring mental health and substance use disorders.
- Strong clinical interviewing and assessment skills.
- Knowledge of ASAM Criteria and levels of care.
- Excellent written and verbal communication skills.
- Experience with Electronic Health Records (EHRs).
Preferred
- Massachusetts BSAS experience.
- Residential treatment experience.
- Experience using InSync.
- Knowledge of Massachusetts behavioral health systems and referral networks.
