About Us

Together Towards Independence (TTI) partners with the Florida Agency for Persons with Disabilities (APD) to empower individuals with developmental disabilities to live meaningful, self-directed lives. We provide compassionate, person-centered support coordination under the APD Home and Community-Based Services (HCBS) Waiver.

Position Overview

The Support Coordinator serves as the central link between individuals, families, service providers, and APD. You’ll coordinate supports, advocate for client rights, and ensure services are delivered according to each person’s Support Plan and APD standards.

What You’ll Do

Develop and monitor person-centered Support Plans and Cost Plans.
Maintain regular contact (monthly/quarterly) with clients and families.
Coordinate and monitor all Medicaid waiver services and authorizations.
Document case notes, summaries, and annual reviews in compliance with APD rules.
Facilitate annual Support Plan meetings and goal reviews.
Respond promptly to incidents, crises, and service concerns.
Ensure accurate, timely documentation for billing and audits.
Advocate for independence, inclusion, and self-determination.

Qualifications (APD Required)

To qualify as a Support Coordinator under APD Rule 65G-13 and the iBudget Waiver Handbook, applicants must meet the following:

A bachelor’s degree from an accredited college or university.
Two years of paid, supervised experience in developmental disabilities, special education, mental health, counseling, guidance, social work, or health and rehabilitative services. A master’s degree in a related field can substitute for one year of the required experience.

Additional Requirements: Completion of APD Support Coordinator Certification (or ability to obtain), Level 2 background screening (Chapter 435 F.S.), valid Florida driver’s license and reliable transportation, and working knowledge of HIPAA, client rights, and Medicaid compliance.

Skills and Competencies

Strong knowledge of APD and Medicaid documentation standards.
Excellent organization, time-management, and written communication skills.
Person-centered planning and advocacy skills.
Ability to build trust with families, providers, and interdisciplinary teams.
Attention to detail and high ethical standards.
Proficiency with Microsoft Office and iConnect (or similar case-management system).

Preferred Experience

Prior work in Support Coordination, Case Management, or Social Services.
Experience supporting individuals with intellectual or developmental disabilities.
Knowledge of the iBudget Waiver, community resources, and Florida’s service network.

Work Environment

Field/office position. 2 days per week required in office. The other three in field seeing clients.
Travel within assigned region for home visits, provider meetings, and plan reviews.
