Clinical Services Manager

Department(s): Primary Care

Reports to: Center Director

FLSA status: Exempt

Job Summary

Under the direction of the Center Director, the Clinical Services Manager is responsible for ensuring leadership of the AgeWell PACE Clinic, providing leadership and direction to the nursing department (clinic and home care), and integrating this department's leadership and functions with those of the other departments to fulfill AgeWell PACE’s mission. This position provides direct supervision to AgeWell’s team of RN Case Managers, Medical Assistants, Schedulers, and Home Care Coordinator(s)

Position Responsibilities

Clinical Oversight

Coordinate the daily workflow of tasks/duties and articulate needs appropriately to the team; this includes working with Clinic RN’s, Medical Assistants, Schedulers, and Home Care.
Lead daily clinic huddles and problem solve issues in a timely fashion.
Collaborate with program management to ensure appropriate staffing.

Clinic & Home Care Services & Quality Assurance

Demonstrate a strong understanding and commitment to PACE, and other applicable regulatory requirements as stated in the policies and procedures; lead effectively ensure adherence.
On an ongoing basis, plan, implement and evaluate clinic and home care service delivery operations in the clinic and home-based settings.
Develop and achieve written goals and objectives for the department. Assure quality standards are met or exceeded. Ensure effective monitoring of key performance indicators and standards. Incorporate high satisfaction rates of participants, families, and employees as a measure in monitoring staff performance.
Be actively involved in program operations to ensure that staff achieves stated goals and operational targets. Appropriately delegate responsibilities and functions to those directly supervised, allowing them the latitude to identify the ways and means of accomplishing goals.
Understand regulations and promote and ensure compliance with all Medicare, Medicaid and state facility and health plan licensing standards applicable to AgeWell PACE. Respond to deficiencies of site audits when necessary and implement and monitor corrective action plans.
Standardize, implement, and oversee policies, procedures, and workflows.

Service Development

Evaluate operational systems and fine-tune or create new procedures, bringing an analytical orientation to daily clinic operations.
Implement the best practice approach and ensure staff are trained to provide services effectively and efficiently.
Anticipate and respond creatively to new service demands in the marketplace (consumer preference or regulatory changes) for the nursing and home care departments. Provide leadership to facilitate change management processes. Serve as a sounding board for innovative ideas and approaches.
Align departmental goals with organizational goals and support necessary operational changes.

Staff Development

Provide for continuity of care by recruiting, developing and motivating employees to maintain high employee morale and minimize turnover.
Create and support a learning environment and ensure staff competencies and training goals are met. Encourage professional and personal development and use regular ongoing training sessions to improve quality and productivity.
Provide oversight, development, and monitoring of staff’s performance within their discipline and on the team. Provide direction, mentoring, guidance, and support to ensure quality service delivery.

Site Leadership Team

Provide leadership to the nursing, clinic, and home care departments.
Collaborate with other members of the AgeWell PACE leadership team on process improvement initiatives.
Participate in long-range planning and special projects to bring a nursing perspective to work plans.
Participate in the preparation of annual budgets for the health center and home care departments. Effectively manage resources, including tracking expenses vs. budget and reviewing invoices for accuracy.
Collaborate with colleagues and staff to develop consensus and negotiate solutions consistent with AgeWell PACE’s mission, business, and operational goals. Actively promote a learning organization by encouraging open communication.
Collaborate with other leaders and administrative staff to promote positive relationships.

Experience & Education

Associates or Bachelor’s degree in Nursing
Master’s Degree in prepared health related field (preferred)
At least 3 years’ experience in Management / Leadership experience in clinic or home care setting.
Minimum of one year’s experience working with the elderly or frail population required.
Proven history of management and leadership of nursing, clinic, and home care staff.
Demonstrated ability to maintain an upbeat, positive approach to managing, reinforcing positive behaviors, and creating highly effective teams.
Skilled facilitator, mentor, coach, and problem solver with excellent communication and interpersonal skills.
Computer literate in Microsoft Office and Electronic Health Records.
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.

Physical demands: Employee must be able to sit for extended periods of time. Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for daily interactions with staff and elderly participants.

Work Environment: Employee must be able to work in a busy and demanding clinic environment with moderate to loud noise levels.
