Full Job Description

Holy Family Home (Little Sisters of the Poor) is a Long-Term Care facility that is dedicated to caring for the elders. Holy Family Home provides our residents with high quality and caring services. Holy Family Home is responsible for the care, comfort and safety of our residents in all activities of daily living. Holy Family Home is dedicated to treating our patients and each other as family as well as ensuring we provide a comfortable and friendly place to work.

Holy Family Home is looking for an amazing experience RN MDS Coordinator or RNAC to join our family team for a <20 bed Skilled Nursing facility.

Position requires you to be Up-To-Date flu vaccinated.

JOB REQUIREMENTS:

Oversees the initiation and completion of the comprehensive assessment and plan of care to assure that the necessary services to attain or maintain the highest practicable mental and psychosocial wellbeing of the Residents is met. These services are consistent with the Home's standards and philosophy and comply with all regulations, codes, policies and procedures that govern the facility.

JOB RESPONSIBILITIES

No Short-Term Care - Long-Term Care Only.
Schedules, organizes and participates in weekly care conference meetings to assure documentation and updated information is accurate and reflective of MDS review.
Schedules, coordinates and oversees the completion of Resident assessment to include: MDS, CAAS, quarterly and annual reviews, initiates significant changes as warranted. assures that information and signatures are obtained in a timely manner from all disciplines involved in care.
Performs weekly transmissions to the state and updates 802 reports.
Maintains confidentiality of all Resident, departmental and organizational information. Demonstrate behavior that indicates recognition of Resident rights.
Discusses with therapy manager on a daily basis to monitor Residents’ status, maintain Med A Residents’ RUGs and be aware of Residents who have begun therapy and/or discharged from therapy.
Works effectively and cooperatively as a team member with Residents and their families, departments and other disciplines.
Participates in UMR reviews and State surveys.
Initiates notification letters for therapy start of care, need for 48-hour notices to be sent by Social Services and discontinuation for therapy.
Perform other duties as assigned by supervisor.

JOB QUALIFICATIONS:

Must be a graduate of an accredited School of Registered Nursing, licensed to practice in Pennsylvania.
Must have training or experience in areas such as nursing administration/supervision, rehabilitation nursing, psychiatric or geriatric nursing.
Must have leadership, self-reliance, creativity, integrity, initiative, good judgment, dependability, good communication and documentation skills.
Must possess an ability to work congenially with others, possessing a spirit of cooperation and enthusiasm in order to create an atmosphere conducive to Resident rehabilitation and staff growth.
Must have background experience or knowledge of Federal, State and local codes and regulations governing long term care facilities and MDS as applicable to job objectives
Must have a basic knowledge of Federal, State and community resources or the ability to pool from appropriate resources, information applicable to job objectives.
Must possess good written and verbal communication skills and knowledge of Point Click Care software program. Must be able to read, write and understand English.
Must possess the ability to perform *essential job functions with or without reasonable accommodations.

MISCELLANEOUS

Maintain CONFIDENTIALITY of all Home information.
Follows all safety and security policies and procedures.
Observe dress and conduct codes, including wearing identification badge.
Meet attendance standards as established and provide proper notice of absence.
Complete duties in prescribed time.
Attend in-services and department meetings.
Perform other duties as assigned by supervisor
Able to perform simple motor skills such as standing, walking, sitting, etc.
Able to remember simple, complex and multiple tasks and assignments given to self and others ranging from short to long periods of time. Must be able to concentrate on moderate to fine detail with constant interruption. Must attend to task/function for more than 60 minutes at a time.
Must be able to communicate effectively in English.
Must have the cognitive ability to constantly understand theories behind several related concepts, comprehend, remember and follow multiple task/directions over long periods of time.
Must be able to educate staff.
Must be able to act in a professional manner at all times.
Must be able to keep abreast of ongoing changes to the MDS process and current through CMS.
Must be able to multi-task

WORK SETTING/ENVIRONMENT:

Work is performed primarily indoors but at times may attend meetings outside the facility. Work environment is well lighted, clean and suitable to tasks performed by the employee within temperature control standards that govern a skilled care facility.
