Full Job Description

Job Title: Family and Community Navigator

Classification: Non-Exempt Full-time

Department: Catholic Charities of Philadelphia-Family & Community Services

Reports to: Administrator

Direct reports: volunteers and/or student interns as appropriate

Summary

The Family and Community Navigator will provide comprehensive case management services to vulnerable community members seeking assistance with a wide range of social service needs with priority given to pregnant women and mothers of young children. Utilizing a strengths-based and trauma informed lens, the candidate will conduct intake screenings and assessments, offer individualized navigation support, and connect to services within the Catholic Charities of Philadelphia (CCoP) network and throughout the broader social service community. The Navigator will coordinate and facilitate on-site pantries in the catchment area for which services are provided. Wraparound support to clients will promote stability and self‑sufficiency.

Principal Duties and Responsibilities:

· Conduct initial screenings with community members to determine service needs and the level of support required.

· Provide comprehensive one‑on‑one case management services to an assigned caseload, including intake and assessment, goal planning, referrals and linkage to community resources, and ongoing follow‑up through meetings and visits.

· Maintains an active caseload of approximately 25-50 households depending upon the complexity and needs of the program. Intensive case management services for higher acuity needs and short-term assistance to lower-acuity clients.

· Builds and maintains professional, supportive relationships to ensure effective advocacy, guidance, and empowerment for community members working toward increased stability and self‑sufficiency.

· Plans and coordinates pantry visits for the community within the catchment service area for items such as diapers, baby items, and hygiene products.

· Assists Administrator and CCoP staff in the development of coordination of donations for pantries.

· Maintains accurate and organized documentation of all services provided in a timely manner, including maintaining up‑to‑date client records in the Apricot database.

· Participates in weekly supervision sessions for case consultation, professional development, and program alignment.

· Collaborates with CCoP colleagues on community initiatives and events and participates in regularly scheduled Family Navigation Meetings.

· Attends staff meetings, CCoP trainings, and funder‑required meetings and trainings.

· Adheres to requirements outlined in the grantee response for funding provided by the Pennsylvania Department of Human Services, William Penn Foundation and any other private or public funding sources.

· Performs other tasks and duties as assigned.

Knowledge, Skills, and Abilities:

· Knowledge of and orientation to the philosophy of Catholic Charities of Philadelphia as presented in the Mission Statement.

· Adherence to the teachings of the Catholic Church especially related to the position on abortion and contraception.

· Knowledge of crisis intervention techniques, child abuse reporting, de-escalation techniques and trauma informed care.

· Ability to maintain confidentiality regarding client information adhering to policies and procedures with knowledge of HIPAA laws.

· Ability to establish and maintain a positive and professional relationship with co-workers, clients, and visitors.

· Demonstrate cultural sensitivity and ability to relate well and effectively in a diverse workplace and with a diverse client population

· Knowledge of current social service resources and delivery systems to meet a wide range of client needs.

· Access to and ability to utilize translation services to clients for whom English is not their first language.

· Ability to organize, efficiently document, and routinely update information in online reporting systems.

· Proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint) as well as online platforms such as Zoom, Microsoft Teams and others for online meetings, training and workshops.

· Requires access to and use of an automobile for timely provision of community-based services.

· Ability to work in person Monday-Friday with some occasional evening/weekend hours required.

Education

· Bachelor’s Degree in Social Work or related field of study

Experience

· 1 – 3 years of professional experience

· Bilingual or multilingual abilities (English/Spanish or English/ French strongly preferred)

Physical Requirements

· Must be mobile and able to bend, stoop and moderately life (up to 30 pounds)

· Must be able to sit/stand at a workstation or computer screen for up to 2 hours at a time.

· Must be able to sit/stand at both indoors and outdoors events for up to 2 hours at a time.

Pay: From $45,000.00 per year
