Full Job Description

Job Overview

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide Respite Nursing (HNR) services within our Home and Community-Based Care program.

In this role, you will support individuals with developmental or medical needs by providing skilled nursing care in the home while offering temporary relief to families and primary caregivers. This position is ideal for nurses seeking flexible, shift-based scheduling while making a meaningful impact in the community.

At this time, full-time positions are not available.

Position Type

Part-Time / PRN / Full-time
Shifts available: 4-hour, 6-hour, and 12-hour shifts
Schedule based on patient authorization and coverage needs
Pay rate: Up to $37.00/visit

Responsibilities

Provide skilled nursing care in the patient’s home under HNR (Respite) services
Follow physician orders and established plans of care
Monitor patient condition and report changes to the supervising RN
Support patients with daily medical needs while maintaining safety and comfort
Provide caregiver relief during authorized respite periods
Maintain accurate and timely clinical documentation
Communicate effectively with the care team and administrative staff
Adhere to all state regulations, company policies, and patient confidentiality standards

Qualifications

Active Arizona LPN license required
Current BLS/CPR certification required
Fingerprint Clearance Card (or ability to obtain) required
Experience working with individuals receiving services through the Arizona Division of Developmental Disabilities (DDD) preferred
Article 9 Certification (or willingness to complete) required
Reliable transportation and active vehicle insurance required
Ability to work independently in a home-based setting
Strong communication, time management, and documentation skills

Additional Information

Orientation, onboarding, and shadowing required prior to independent shifts
Continued compliance with licensing and certification requirements is mandatory

Job Types: Part-time, PRN, Travel nursing

Pay: From $37.00 per hour

Expected hours: No more than 40 per week
