Full Job Description

Job Summary
Father Matters is seeking a compassionate, organized, and employment-focused Case Manager to provide comprehensive case management and workforce services to veterans participating in the Father Matters Veterans Workforce Advancement Program. The program serves veterans to experience or at risk of homelessness and helps participants overcome employment barriers, access training and supportive services, obtain meaningful employment, and achieve long-term stability.

The Case Manager will work directly with veterans using a client-centered, strengths-based, and trauma-informed approach. This position is responsible for eligibility screening, enrollment, assessment, employment planning, service coordination, career development, job placement support, employment retention, and follow-up.

Duties

· Travel into the community to recruit eligible participants.

· Conduct outreach activities and classes to recruit eligible participants in Lewis and Perryville Prison and Maricopa County Sheriff’s Office Jails.

· Conduct intake, eligibility screening, enrollment, and comprehensive assessments for veterans seeking program services Conduct outreach and follow-up with reentry participants to assess progress, identify needs, and track client outcomes.

· Collect and maintain required documentation to establish HVRP eligibility, including documentation of veteran status and homelessness, risk of homelessness, participation in a qualifying partner program, or transition from an eligible institution.

· Will ensure participant eligibility and enrollment documentation is accurate and complete before HVRP-funded participant services are provided Identify appropriate community resources and make referrals to partner organizations when additional services are needed.

· Develop an Individual Employment Plan (IEP) with every enrolled participant Facilitate workshops, job clubs, classes, and other group activities as needed.

· Will regularly review and update the IEP as the participant progresses toward employment and self-sufficiency.

· Provide ongoing employment-focused case management, career coaching, and barrier-resolution services. Responsibilities include helping participants identify career pathways, prepare resumes and applications, strengthen interviewing and workplace-readiness skills, conduct job searches, and connect with employers.

· Will coordinate with the Employer Outreach Coordinator to connect participants to job openings, On-the-Job Training, Registered Apprenticeships, occupational training, and other work-based learning opportunities aligned with local labor market demand

· Assess supportive service needs that may interfere with employment or training, including transportation, childcare, work clothing and tools, identification documents, housing instability, behavioral health needs, legal barriers, and other employment-related challenges

· When appropriate, the Case Manager will connect participants with community resources Refer clients to qualified legal advisors or other legal resources when legal advice or representation is needed.

· Will coordinate referrals and services with American Job Centers, VA programs, housing providers, behavioral health providers, correctional and reentry agencies, education and training organizations, Veteran Service Organizations, and other community partners.

· Maintain timely and accurate case notes, intake information, client records, referrals, and outcomes in the organization's case management system.

· Will maintain regular contact with participants throughout program participation and provide employment adjustment and retention services after placement. Follow-up may include assistance with workplace challenges, transportation, scheduling, financial stability, workplace communication, employer expectations, and other issues that could affect job retention.

· Participate in staff meetings, training, and professional development activities.

Minimum Qualifications

· Valid driver’s license, reliable vehicle, and current auto insurance

· No history of sex offenses or convictions related to child/adult abuse or neglect

· High school diploma or GED required; associate or bachelor’s degree in social work, human services, workforce development, behavioral health, criminal justice, or a related field preferred.

· Must be able to be badged for ADCRR and MCSO Jails.

· Ability to work effectively with veterans experiencing homelessness, housing instability, unemployment, justice involvement, and other employment barriers.

· Strong communication, interviewing, problem-solving, and relationship-building skills.

· Ability to develop professional boundaries while building trusting relationships with participants.

· Strong organizational skills and ability to maintain accurate and timely case documentation.

· Basic computer proficiency and ability to learn electronic case management and federal reporting systems.
