Behavioral Health/Long Term Care - Skilled Nursing Facility

We are seeking compassionate, dependable, and clinically skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our Skilled Nursing Facility. Our team provides exceptional nursing care to residents with complex medical and behavioral health needs in a safe, therapeutic, and supportive environment.

Position Summary

The LPN/RN is responsible for providing high-quality nursing care, administering medications and treatments, monitoring residents' physical and mental health status, and collaborating with an interdisciplinary team to promote optimal outcomes. This role requires strong clinical judgment, excellent communication skills, and the ability to remain calm and professional in challenging situations.

Responsibilities

Provide direct nursing care in accordance with physician orders, facility policies, and state regulations.
Assess, monitor, and document residents' physical and behavioral health status.
Administer medications, treatments, and injections accurately and safely.
Respond appropriately to changes in resident condition and medical emergencies.
Collaborate with physicians, psychiatric providers, therapists, and interdisciplinary team members.
Develop and update individualized care plans.
Promote a therapeutic, respectful, and trauma-informed environment.
Supervise and support Certified Nursing Assistants (CNAs) and other support staff (as applicable).
Maintain accurate and timely electronic medical records.
Ensure compliance with infection control, safety, and regulatory standards.
Build positive relationships with residents, families, and team members.

QualificationsLPN

Current Licensed Practical Nurse (LPN) license in good standing.
Current CPR/BLS certification.
Fingerprint Clearance Card
Long-term care, skilled nursing, or behavioral health experience preferred.
New graduates with strong clinical skills are encouraged to apply.

Current Registered Nurse (RN) license in good standing.
Current CPR/BLS certification.
Fingerprint Clearance Card
Experience in skilled nursing, psychiatric nursing, behavioral health, or long-term care preferred.
Strong assessment and critical thinking skills.

Preferred Qualifications

Experience caring for residents with mental illness, dementia, substance use disorders or co-occurring medical conditions.
Knowledge of psychiatric medications and behavioral interventions.
Experience with electronic health records (EHR).
CPI or de-escalation training is a plus.
