Full Job Description

A Case Manager works directly with people in addiction treatment or recovery. This work may also involve families and friends of the client. A Case Manager also works with service providers, to engage resources and achieve coordination of care for a client. Basic Purpose: To work directly with clients who are seeking, receiving treatment, or have been identified as having substance abuse challenges. Case Managers/Care Coordinators do not focus on clinical work, but they must still be capable of responding in professional, ethical ways, including cultural responsiveness and trauma-informed best practices.

The Case Manager/Care Coordinator must be able to:

o Work with the client to create a service or recovery plan

o Match the needs with available community services

o Assist people to access services

o Use evidence-based or best practices

o Live up to professional and ethical responsibilities

o Complete necessary administrative tasks

o Advocate for the client with a variety of service providers

QUALIFICATIONS and additional job duties:

Prefer Case Management in related field with one year related experience.
Knowledge of substance abuse and co-occurring disorders – characteristics, assessment, and treatment.
Personal characteristics of maturity and professionalism with a sincere desire to work with recovering individuals.
Basic computer skills in word processing, e-mail, and Internet use.
Must possess a valid State of Ohio driver’s license and possess reliable transportation. (applicants must be insurable under our agency vehicle insurance)

· Communicates effectively both verbally and in writing, establishes positive public relations and interacts effectively with a diverse array of professionals and clients.

· Acts as a positive role model, providing clients with guidance and support.

Demonstrates patience, caring, compassion, and sensitivity toward clients.
Functions in a cost-effective manner with regard to the utilization of time, supplies and other agency resources.
Attends meetings and supervisions as required and necessary for the position.
Promotes and fosters effective relations, both inside and outside of the agency.
May perform other related duties as required.
