Full Job Description

JOB SUMMARY:

The Breast Care Navigator is an integral member of the Imaging Department and the multidisciplinary breast care team providing patient navigation services. This role focuses on guiding patients with abnormal breast imaging or suspected breast disease through the diagnostic phase of care, ensuring timely access to diagnostic services, clear communication, patient education, and barrier resolution.

The Breast Care Navigator supports patients from initial abnormal screening through definitive diagnosis and coordinates a seamless transition to the Oncology care team once a cancer diagnosis is established.

ESSENTIAL FUNCTIONS:

Serve as the primary point of contact for patients with abnormal breast imaging or suspected breast disease during the diagnostic phase.
Coordinate and facilitate timely diagnostic breast imaging, biopsies, and pathological results.
Collaborate closely with radiologists and referring providers to reduce delays in diagnosis and treatment planning.
Assures access to supportive care.
Engage patients and family members to ensure understanding of the diagnostic pathway and expectations.
Act as a patient advocate within the healthcare system to support timely, patient-centered diagnostic care.
Serve as a liaison between the Imaging Department and the multidisciplinary breast care team.
Ensure patients understand the next steps in care and appropriate Oncology referrals are initiated.
Maintain accurate and timely documentation in the electronic health record.
Adheres to dress code.
Adheres to medical center policies addressing infection control, safety and personnel.
Maintains patient confidentiality and treats patients and their families with respect and dignity.
Functions as a preceptor to new employees.
Active member of Breast tumor board.
Performs other related duties as assigned or requested.

JOB REQUIREMENTS:

1. Interpersonal skills.

2. Experience with working with oncology patients and families are preferred.

3. Knowledge of the breadth and accessibility of community resources.

4. Thorough knowledge of health care systems and terminology.

5. Working knowledge of HIPPA regulations.

6. Self directed and able to organize and manage multiple tasks/projects simultaneously.

7. Ability to promote programs and services to community.

8. Extensive understanding of oncology care.

9. Proficient in learning medical record systems (electronic and non-electronic).

10. Written and verbal communication skills.

EDUCATION AND LICENSURE:

An Associate’s Degree in Nursing is required. Bachelor’s Degree in Nursing is preferred. Current Kentucky RN license is required. BLS training is required. Three or more years of experience, preferable with oncology patients.

WORKING CONDITIONS:

Works in well-lighted business office environment the majority of the time

There is a potential for being at risk of exposure to hazardous chemicals when working. Information regarding hazards and protective measures along with training on handling and storage of hazardous materials will be provided prior to initial assignment to work with hazardous chemicals, whenever the hazard changes, and on an annual basis

Required:

An Associate’s Degree in Nursing is required.

Current Kentucky RN license is required.
