Overview

We are seeking a highly skilled and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will possess a comprehensive understanding of clinical documentation standards, Medicare and Medicaid regulations, and long-term care requirements. In this pivotal role, you will oversee the accurate completion and timely submission of the Minimum Data Set (MDS) assessments, ensuring compliance with federal and state healthcare policies. Your expertise in medical coding, utilization management, and electronic health record systems will support optimal patient care and facility reimbursement processes. This position offers an opportunity to contribute to quality improvement initiatives within a dynamic healthcare environment committed to excellence in patient outcomes.

Responsibilities

Coordinate and facilitate the completion of MDS assessments for long-term care residents, ensuring accuracy and compliance with CMS regulations.
Review medical records, clinical documentation, and coding details such as ICD-10, CPT, and DRG codes to support precise data entry and billing processes.
Collaborate with interdisciplinary teams—including nursing, therapy, social services, and case management—to gather comprehensive resident information for assessment accuracy.
Ensure adherence to documentation standards aligned with HIPAA, NCQA standards, and state healthcare regulations while maintaining confidentiality of protected health information (PHI).
Utilize EMR/EHR systems such as Cerner or eClinicalWorks to document assessments, track progress notes, and manage electronic health records efficiently.
Conduct ongoing audits of medical documentation to identify gaps or inconsistencies; provide education on clinical documentation improvement strategies.
Support discharge planning, hospice care coordination, and post-acute care transitions by providing accurate data for appropriate level-of-care determinations.
Stay informed about updates in Medicare regulations, long-term care policies, and CMS guidelines to ensure ongoing compliance across all documentation activities.

Experience

Proven experience in long-term care facilities or nursing homes with a strong understanding of MDS processes and regulations.
Demonstrated knowledge of ICD-10/ICD-9 coding systems, CPT coding procedures, DRG assignments, and utilization review practices.
Prior experience with electronic health record (EHR) systems such as Cerner or eClinicalWorks is highly desirable.
Background in clinical documentation improvement (CDI), case management, or health information management preferred.
Familiarity with Medicare regulations, CMS guidelines, NCQA standards, and state healthcare policies governing long-term care settings.
Experience working within hospital or inpatient environments with acute care or ICU exposure is advantageous.
Strong understanding of medical terminology, anatomy, physiology, and nursing practices related to patient care documentation.
Excellent organizational skills coupled with the ability to prioritize tasks effectively within fast-paced healthcare settings.
