Full Job Description

This is not a traditional NP role — and it is not a traditional leadership role. This is an opportunity to do both, simultaneously, from the comfort of your home office.

As a Clinical Lead Nurse Practitioner you will oversee a clinical outreach program dedicated to managing some of the most complex and vulnerable patient populations — Veterans, Medicaid, and Dual Eligible members — while leading and mentoring a team of nurses who are coordinating care on the front lines every day.

If you are an experienced NP who is ready to step into a senior clinical leadership role without giving up clinical relevance, patient impact, or the flexibility of remote work — this role was built for you.

What You'll Do

As Clinical Lead NP you will build and review individualized care plans for complex patients, oversee clinical nurse outreach programs to ensure timely, high-quality patient engagement, handle clinical escalations and complex patient scenarios with confidence and decisiveness, collaborate with nurses, care coordinators, specialists, pharmacists, and other healthcare professionals to optimize patient outcomes, partner with the Chief Medical Officer and clinical staff to develop and enhance clinical operations, partner with the Director of Care Coordination to ensure effective team onboarding and ongoing staff development, advocate for and support patients in managing their health and healthcare journey, build trust with members, families, and external partners through active listening, intentional communication, and compassionate problem-solving, and use motivational interviewing techniques to influence positive health outcomes.

What You'll Need

Required:

Active, unrestricted Nurse Practitioner license in Texas — required
Ability to obtain additional state licenses as needed
Board certification from ANCC, AANPCB, or other nationally recognized certifying body — required
Graduate of an ACEN or CCNE accredited nursing program — required
5+ years of progressive leadership experience managing a clinical team — required
2+ years of remote work experience — required
Experience in Chronic Care Management — required
Experience working with Veterans and/or Medicaid and/or Dual Eligible patients — required
Dedicated home office — required
Strong interpersonal, verbal and written communication, and problem-solving skills
Experience working with patients via phone and text message
Experience documenting in care management or electronic health record platforms
Experience with motivational interviewing

Preferred:

Additional licensure in one or more of the following states: Louisiana | Mississippi | Arkansas | Oklahoma | Missouri | Kansas | Nebraska | Utah | Wyoming | Colorado
Provider type: FNP | ANP | PMHNP | CNS
Bilingual fluency in English and Spanish
Experience with Apple computers and MacBook
