Full Job Description

Position Overview and Purpose

The “Program Specialist” is responsible for all assigned individuals residing in community homes and is responsible for health and safety, general growth, development and wellbeing of these individuals. The Program Specialist will participate in the development, and oversee implementation of the ISP, act as a liaison between the organization and county, state or federal entities requesting individual information. Ability to communicate with individuals, staff, medical offices and professionals, supervisors, managers and families. The Program Specialist will serve as the agency designee for Incident management, state licensing and program monitoring. The Program Specialist is responsible for ensuring all documentation regarding care of individuals Is adequately maintained and filed pursuant to all governing regulations

Essential Job Functions

· Maintain positive and effective relationships with families, other care providers, ISC;s and any county, state or federal representative along with the overall community

Serve as WRC’s liaison to SCO’s, county, state and federal agencies, day programs, support services, doctors, family members
Assist the Chief Operations Officer in developing SOP's directly related to residential supports and provide training to program staff as needed
Participate on all committees and administrative meetings as assigned
Participate in new hire training
Ensure all consumer books are well maintained and meet county, state and federal regulations conduct routine site visits as required
Report all emergency and non-emergency repair needs through work order process
Participate in the Incident Management process and the filing of incidents in EIM as directed
Ensure all tracking forms and goal sheets are kept up-to-date in the community homes and that all issues or changes needed in goals are reported to all applicable team members
Participate in the development, writing, implementation and revisions of Outcomes, Behavioral Support Plan, SEE Plan, Fade Plan, Transition Plan and Restrictive Plan. Additionally, Program Specialist will provide training to all involved program staff on these plans.
Collect data from all applicable forms and plans listed above on a weekly and monthly basis, monitor trends and share data and participate in revisions as needed
Work with Medical Department to schedule all individual’s annual medical appointments and ensure they are completed within the time frames stated in regulations as well as in their ISP.
Successfully complete Medication Administration Training
Coordinates the transition of new consumer as required
Provide care to all consumers as required including crisis intervention
Receive changes made to the individual’s inventory lists and record all changes on the master lists
Coordinate and complete all assessments required by county and state regulations for new individuals and on an annual basis
Participate in the development of the ISP, ISP annual update and ISP revisions as needed
Provide documentation and updated information to SCO's
Participation in ISP meetings, fulfill the role of plan lead as applicable and inform plan team members of the option to decline the ISP review
Participate in all Interdisciplinary team meeting as directed in the ISP
Throughout the year review the ISP, report all changes to SCO's and team members
Implement the ISP as written
Review ISP's with consumers and document the review of the ISP as required
Oversee Managers and program staff working in community homes, monitor house managers and program staff’s performance, and provide training when necessary
Work hand in hand with other employees working directly with WRC consumers (i.e. behavioral specialist, speech therapists, etc.)
Participate in the hiring and termination process as deemed necessary by Human Resources and COO
Assist Program Coordinators in team meetings.
Support consumers in securing financial guardians and/or rep. payees as needed.
Responsible for notification and follow through for all consumers address and budget changes to all involved parties, the county and state when necessary
Participates in the licensing of all new community homes as needed
Participates in HRST entry and oversight as required
Ensure adherence to agency, county, state, federal and all other regulations
Other duties as assigned

Qualifications:

A. A masters degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism

B. A Bachelor’s degree from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability or autism

C. An associated degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with individuals with an intellectual disability or autism

D. Proficient in Microsoft Office

E. Valid Pennsylvania driver’s license. Use of personal vehicle required.
