Director of Human Resources – Freeport & Brookville

Full-Time

Salary: $125K - $135K/yr

Advantage Care Health Centers and The Fay J. Lindner Center for Autism are Long Island’s premier Federally Qualified Health Care Centers (FQHC). Located in Brookville and Freeport, NY, we provide comprehensive medical, dental, and mental/behavioral health services to children and adults.

The mission of Advantage Care Health Centers is to provide the highest quality comprehensive primary, dental, mental, and behavioral health care services.

The Director of Human Resources is a member of the senior leadership team, responsible for oversight of all Human Resources functions at Advantage Care Health Centers (“Center”), including but not limited to Talent Acquisition, Onboarding, Training & Professional Development, Employee Relations and Engagement, Leave of Absence, Employer-Sponsored Health & Wellness and Retirement Plans, Compensation/Total Rewards, and Employee Records.

The Director is responsible for developing and executing Human Resources processes in alignment with the Center’s mission, vision, core values, and strategic goals.

In addition, you will:

Direct all Human Resources functions, including talent acquisition, onboarding, employee relations, compensation, benefits, compliance, HRIS, and employee engagement.
Ensure compliance with federal, state, and local employment laws and FQHC/Article 28 regulatory requirements; develop and maintain HR policies, procedures, and the employee handbook.
Lead end-to-end talent acquisition, workforce planning, onboarding, and professional development programs to attract, develop, and retain a high-performing workforce.
Oversee total rewards, health and retirement benefits, wellness initiatives, and compensation strategies to support recruitment, retention, and employee satisfaction.
Manage employee relations, performance management, investigations, corrective actions, and labor matters while minimizing organizational risk and supporting leadership.
Partner with executive leadership, external vendors, and strategic stakeholders to advance organizational goals, optimize HR operations, leverage workforce analytics, and represent the organization professionally.

In this role, you will:

Provide comprehensive primary care to adult and pediatric patients, including physical exams, wellness visits, preventive care, diagnosis, treatment, and medication management under physician collaboration.
Order, interpret, and evaluate diagnostic tests; document clinical findings, develop treatment plans, and monitor patient progress through individualized care plans (PCMH and CCM).
Coordinate patient referrals, follow-up care, and access to specialty providers, community health resources, and social service agencies to ensure continuity of care.
Counsel patients on health management, establish individualized goals, and implement behavior change strategies to improve health outcomes.
Support clinical operations through oversight of scheduling, medical record documentation, chart quality, and development of clinical forms, reports, and documentation procedures.
Collaborate with leadership to provide staff education, promote quality improvement initiatives, and ensure compliance with infection prevention, PPE, and safety protocols.

We require:

Bachelor's degree required (Master's preferred) with a senior HR credential (e.g., SHRM-SCP, PHR, CEBS).
Five (5) or more years of progressive HR leadership experience (10+ years preferred), including healthcare or long-term care; FQHC experience strongly preferred.
Demonstrated expertise across core HR functions with a proven record of leading organization-wide initiatives and achieving measurable results.
Experience using HR metrics and workforce analytics to drive strategic decision-making preferred.
Exceptional leadership, organizational, interpersonal, and written/verbal communication skills.
