Community Health Worker & Family Practice Clinic Manager
POSITION SUMMARY
The Community Health Worker & Family Practice Clinic Manager for the Briscoe County Rural Health Clinic serves a dual role by providing community-based outreach and patient navigation services while overseeing the daily operations of a family medicine clinic. This position acts as a liaison between healthcare providers, social service agencies, and the community to improve access to healthcare and supportive resources. Simultaneously, the position is responsible for managing clinic operations, personnel, compliance, quality improvement initiatives, fiscal reporting, and the overall patient experience.
The incumbent works collaboratively with healthcare providers, staff, patients, community organizations, and external partners to promote high-quality, patient-centered care and improve health outcomes.
ESSENTIAL DUTIES and RESPONSIBILITIES
Community Health Worker Responsibilities
Serve as a liaison between patients, healthcare providers, social service agencies, and community organizations.
Conduct outreach activities to identify community needs and connect individuals and families with healthcare and social service resources.
Assist patients in navigating healthcare systems, insurance programs, referrals, and community services.
Provide culturally appropriate health education, disease prevention information, and wellness promotion activities.
Assess barriers to healthcare access and work with patients to develop strategies for overcoming those barriers.
Advocate for patients and families to ensure they receive appropriate services and support.
Develop and maintain partnerships with community agencies, schools, faith-based organizations, and other stakeholders.
Facilitate enrollment in available assistance programs and community resources.
Maintain accurate records of outreach activities, referrals, and patient interactions.
Participate in community events and health promotion initiatives to increase awareness of available services.
Clinic Management Responsibilities
Oversee the daily operations of the family medicine clinic to ensure efficient and effective service delivery.
Supervise, train, schedule, and evaluate clinic personnel, including administrative and support staff.
Foster a positive work environment that promotes teamwork, accountability, and excellent customer service.
Ensure compliance with all federal, state, local, and organizational regulations, policies, and procedures.
Monitor clinic performance and implement quality improvement initiatives to enhance patient care and operational efficiency.
Collaborate with healthcare providers to support clinical operations and continuity of care.
Oversee patient scheduling, registration, referrals, and workflow processes.
Manage patient complaints, concerns, and service recovery efforts to ensure a positive patient experience.
Coordinate facility operations, equipment maintenance, and supply management.
Prepare and submit required reports related to clinic operations, productivity, and performance metrics.
Assist with budget development, fiscal reporting, and monitoring of clinic expenditures.
Support strategic planning and program development efforts.
Maintain confidentiality and security of patient and organizational information in accordance with applicable regulations.
Leadership and Collaboration
Serve as a resource and advocate for patients, families, staff, and community partners.
Promote patient-centered care and health equity initiatives.
Participate in interdisciplinary team meetings and organizational committees.
Support organizational goals related to quality, safety, compliance, and community engagement.
Represent the clinic in community partnerships and collaborative initiatives.
REQUIRED QUALIFICATIONS
High school diploma or equivalent required.
Community Health Worker Certification (can be obtained within 6 months of employment).
Previous supervisory or management experience preferred.
Knowledge of community resources, healthcare systems, and social service programs.
Experience with electronic health records (EHR) systems and Microsoft Office applications.
Excellent interpersonal, communication, organizational, and problem-solving skills.

WORKING CONDITIONS
Work is performed in both clinic and community settings.

May require flexible scheduling, including occasional evenings or weekends, based on organizational needs.
PHYSICAL REQUIREMENTS
Ability to sit, stand, walk, and move throughout the clinic and community settings.
Ability to lift and carry materials up to 25 pounds.
Ability to operate standard office equipment and computer systems.

Dual reporting relationship to Clinic Nurse Practitioner and WJ Mangold Chief Executive Officer (or designated supervisor)
COMPENSATION
Compensation commensurate with experience.
Position may be eligible for sign-on bonus and relocation reimbursement.
