Job Summary

Join Stellar Health Group in Burlington, VT as an MDS Nurse! As a valued member of our clinical team, you will play a critical role in providing high-quality care to our residents and directing excellent clinical outcomes. Our experienced and dedicated team is passionate about creating a warm and welcoming environment for our patients, and we're seeking like-minded professionals to join our family.

Responsibilities

Conduct and coordinate the development and completion of the resident assessment
Conduct resident assessments in accordance with current rules, regulations, and guidelines
Implement resident CAA's
Participate in facility survey readiness and throughout the survey
Work with our interdisciplinary care team to develop a comprehensive assessment and care plan for each resident

Qualifications

Current unencumbered licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Vermont
Two years of MDS and PPS experience
Care Planning experience
