JOB TITLE: Case Manager

DEPARTMENT: Mental Health

SUPERVISOR: Head of Service

SUPERVISES: None

JOB SUMMARY: The Case Manager is responsible for the direction and management of the resident’s plan as indicated through their Child and Family Team. Assists the treatment team in assessing residents and preparing a written Mental Health Treatment Plan, addressing needs according to the assessments. Provides the Intensive Care Coordination component of care for the resident, family, and other members of the Child and Family Team. Interacts with multiple systems and agencies to ensure collaboration between them.

MINIMUM QUALIFICATIONS:

Education:

Bachelor's degree in psychology, social work, childcare counseling, education or development from an accredited college or university is required and/or two of college with four years’ experience working with population and/or be enrolled in college working toward degree and have six years’ experience working with population.

Experience:

One (1) year direct service experience with severely emotionally disturbed children in an inpatient or outpatient health care setting (internship and/or practicum time may be substituted).

One (1) year case management experience preferred.

License or Certification:

Valid California driver's license.

Specialized Skills:

Ability to interact and communicate both verbally and in writing with seriously emotionally disturbed children, other staff, physicians and the public.

Demonstrated understanding of psychiatric illness and its treatment.

Demonstrated ability to visually and audibly assess the mental status and psycho-social needs of severely emotionally disturbed children.

Demonstrated understanding and knowledge of community services and county agencies.

Physical Requirements:

Ability to visually and audibly assess child's behavior and needs.

Ability to sit at a desk and communicate over the telephone for extended periods of time each day, up to 1 hour at a time.

JOB TITLE: Case Manager

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Assists in performing evaluations and periodic re-evaluations of residents and families describing the level of psychosocial impairment, physical health problems, self-care potential, support network availability, adequacy of living arrangements, financial status, employment status and potential training or educational needs.

2. Visually reviews medical, psychosocial, school and other records.

3. Meets with residents, families, treatment staff, placement staff, members of the resident’s Child and Family Team to ensure medically necessary services are delivered.

4. Assists in developing a comprehensive, written Mental Health Treatment Plan identifying and linking the resident and family’s needs and strengths with the life domain goals negotiated with the resident and family.

5. Coordinates services which are to equip the resident and their family in meet treatment and care coordination needs.

6. Ensures that action items from the Child and Family Meeting minutes are conducted and followed up, interacting with county and regulatory partners.

7. Establishing resources and supports for the resident and family during the time the resident is at the facility and afterwards at their community placement.

8. Identifies, assesses and mobilizes resources to ensure the Child and Family Team plan of action is in alignment with the Mental Health Treatment Plan, including interventions with social security, welfare, and health departments and other community agencies and resources as necessary.

9. Conducts referral, linkages, monitoring, and follow-up activities.

10. Makes recommendations to the Child and Family Team members regarding changes to the resident’s plan and works with members to make these changes.

11. Assisting the Head of Service and Therapist in developing the transition plan for the resident and family.

12. Interacts on behalf of the resident with family, friends, legal guardian, and significant others to initiate and maintain supportive networks.

13. As directed, provides Intensive Home Based Services, conduct behavioral analysis and modification skills with family and youth in community.

14. This job description is not intended to be all-inclusive and employee will also perform other reasonably related duties as assigned by the Head of Service or their designee as needed.
