Mount Prospect Academy (MPA) is seeking a compassionate, dedicated, and relationship-focused Permanency Coordinator to join our Pike, NH team. What You'll Do
As a Permanency Coordinator, you will:

Build strong relationships with youth, families, referral sources, and community partners

Facilitate family engagement and strengthen support systems
-
Coordinate family meetings, communication, and treatment planning
-
Attend court hearings and advocate for youth treatment needs
-
Conduct home, school, and community visits
-
Identify and connect families with community resources and support services
-
Provide psychoeducation and parent support
-
Develop home safety plans and intervention strategies
-
Collaborate with clinicians, educators, residential staff, and case managers
-
Complete documentation, progress reports, and discharge planning
-
Support youth and families during and after program discharge through follow-along services
Qualifications
Required:
-
Bachelor's Degree in Social Work, Psychology, Education, Human Services, or related field
-
Minimum 2 years of experience working with emotionally or behaviorally challenged youth, adolescents, or families
-
Valid driver's license
-
Must be 21 years of age or older
-
Ability to successfully complete all required background and pre-employment screenings
