SLS Program Coordinator: Rainbow Home Care

Rainbow Home Care is looking for a dedicated SLS Program Coordinator to provide case management, services, and support to individuals in our Supported Living Services Program, and to help grow the program through new cases and increased service hours. You'll be the primary point of contact between the client, their circle of support, the regional center, and our Personal Attendant (PA) staff — making sure each client gets the services, advocacy, and support they need to live independently and meet their personal goals.

What You'll Do

Coordinate and secure services, resources, and equipment for clients with developmental, behavioral, and physical disabilities, and serve as their advocate and liaison with the regional center, other agencies, and their circle of support.
Conduct intake assessments for new cases and ongoing assessments of client situations; oversee the quality of service delivery and make changes as recommended.
Develop, review, and update each client's Individual Service Plan (ISP) and quarterly Goals and Outcomes, writing goals that are clear, testable, and measurable.
Attend Semi-Annual and Annual IPP Review Meetings, monthly progress meetings, and any impromptu meetings requested by the client, ID team, or regional center service coordinator.
Conduct monthly supervisory visits to each client's home to check safety, review progress on goals, and audit PA documentation.
Train, supervise, and evaluate Personal Attendants according to each client's program plan — including in-service trainings to promote an independent lifestyle, oversight of budgeting/banking and record-keeping tasks, and verbal/written warnings when performance issues arise.
Manage case documentation: Case Notes, Meeting Notes, Log Books, Incident Reports (SIRs) and preventative follow-up, consents and releases, status/follow-up reports, and the client's full "Green File," kept audit-ready for the regional center.
Handle recurring paperwork on the client's behalf as needed (e.g., Medi-Cal/Medicare renewals, rental applications, rental credit, tax documents).
Coordinate Quality Assurance Assessments and new/reassigned/terminated PA staffing requests with Staffing Coordinators and your Supervisor.
Step in on case management issues as they arise — medical changes, staff conflicts, or conflicts within the client's circle of support.
Keep your Program Supervisor updated on client progress, successes, and challenges, and project a professional company image with the regional center and in all client-facing interactions.
Attend regular staff meetings, stay reachable by field phone during work hours, and be willing to go on-call to assist staff with case emergencies.

Qualities We're Looking For

Professional verbal and written communication skills, strong reading comprehension, and excellent follow-through.

Preferred Qualifications

Two (2) years of work experience in behavioral, human services, or health service industries, including experience with special needs populations.
Associate's degree in psychology, therapy, or a qualified equivalent certificate program in mental or physical health careers.
Experience delivering training.
Proficiency in MS Office and general internet use.
Excellent verbal and written English communication skills.
Open availability for full-time work on a flexible schedule.
Valid driver's license, current auto insurance, and daily access to a reliable vehicle; must pass a drug test and TB test.
Reliable transportation and willingness to travel throughout Orange County (North, Central, and South) to serve families; supervisory-visit travel is reimbursed.
