What You’ll Do:

Assist the Program Administrator in overseeing daily program operations and implementation of program plans, policies, and procedures.

Support compliance with all federal, state, and local regulations and agency policies.

Conducts audits of ISPs, utilization reviews, case notes, and related documentation to ensure accuracy and timelines.

Assists with pre-survey reviews and corrective action efforts to maintain audit readiness.

Coordinates with Program Managers to support staff and ensure effective service delivery.

Provides training and ongoing guidance to staff related to person-centered planning and service coordination.

Assists in monitoring services provided to ensure quality, effectiveness, and alignment with client needs.
- Participates in interdisciplinary team meetings and program coordination activities.
- Assists in collecting, tracking, and analyzing program data; prepares reports as required.
- Supports budget monitoring and expenditure tracking in alignment with program allocations.
- Collaborates with community partners, providers, and stakeholders to support program success.
- As needed, manages a limited caseload of clients.
- Participates in meetings, trainings, and other agency activities.
- Performs other related duties as assigned.
Supervisory Responsibilities:

All you need for success:
Minimum Qualifications
Required:
-
Bachelor’s degree in social work (BSW), Human Services, or equivalent human services degree; OR
-
Registered nurse (licensed)
-
Minimum two (2) years of experience in case management or human services.
-
This position requires a valid driver’s license, a reliable vehicle, and proof of insurance.
Preferred:
-
Experience in Developmental Disabilities or Targeted Case Management.
-
Experience in quality assurance, compliance, or program coordination.
-
Supervisory or leadership support experience.
An equivalent combination or relevant education and experience may be substituted as appropriate.
